摘要 [目的]探讨18F脱氧葡萄糖(FDG)PET对小儿难治性癫痫发作期及发作问期癫痫灶的诊断价值。[方法]对23例癫痫患儿于发作期及发作间期行18F-FDG PET检查,并与MRI及EEG比较。男10例,女13例,年龄最小1岁,最大14岁。[结果]23例中91.3%(21例)PET显像有异常代谢灶,低代谢灶19例,高代谢灶2例,2例正常。2例高代谢灶中1例检查时癫痫正发作。24 h后复查仍为高代谢灶。另1例于检查前3 h癫痫发作1次。有2例行手术切除癫痫灶,术中发现病灶及头皮、深部电极癫痫灶与PET异常代谢灶部位相符。另年龄小患儿异常代谢灶呈弥漫改变较年龄大的患儿多。MRI异常率65%(15例),EEG异常率86.9%(20例)。[结论]18F-FDG PET对发作间期或发作期对癫痫灶的诊断价值明显优于MRI和头皮EEG。尤其在发作间期可提高灵敏度和特异性及诊断准确性。 [Objective] To assess the diagnostic value of 18F-FDG imaging on location of epileptic foci during interphase and epileptic attack .[Methods] 23 patients with refractory epilepsy were examined during interphase of epilepsy and epileptic attack by 18F-FDG PET. The results of PET were compared with that of MRI and EEG. Of 23 patients, 10 were male and 13 were female. The youngest was 1 year old and the oldest was 14 years old. [Results] There were some abnormal metabolic foci on PET imaging in 21(91. 3% ) patients. Lower metabolism foci were found in 19 cases. Foci with higher metabolism were found in 2 cases. Two cases were in normal. One of two cases with foci of high metabolism had attack during the examination. There was an epileptic attack 3 h before examination in the other one case. The epileptic foci were removed by operation in two patients. The location of foci with PET was proved to be correct by surgical exploration . Diffuse changes of foci with abnormal metabolism were more common in younger children than in older children , MRI was abnormal in 69% of cases and EEG was abnormal in 86%. [Conclusion] The diagnostic value of 18F-FDG PET imaging on epileptic foci is superior to that of MRI and EEG in terms of sensitivity, specificity and diagnostic accuracy, especially during interphase of epilepsy .
