摘要
目的探讨超声弹性成像面积比(AR)在BI-RADS4类乳腺肿块鉴别中的应用价值。方法选取自2015年7月至2018年10月宣城市人民医院经常规超声检查发现的79例乳腺肿块患者为研究对象,共96个BI-RADS 4类乳腺肿块。计算超声弹性成像图像与常规超声二维灰阶图像的AR,并与病理结果比较。基于病理结果为"金标准"构建受试者工作特征曲线(ROC),以找到诊断BI-RADS 4类乳腺肿块良恶性的最佳截断点。结果经病理诊断,96个BI-RADS 4类乳腺肿块中良性54个,恶性42个。AR为1.095时为最佳诊断截断点,特异度为91.1%,灵敏度为74.5%,诊断符合率为82.3%,ROC曲线下面积为0.848。结论 AR在鉴别BI-RADS 4类乳腺肿块中具有较高的临床应用价值。
Objective To investigate the accuracy of area ratio(AR)on ultrasonographic elastography in diagnosis of BI-RADS 4 breast tumor.Methods A retrospective study was performed on 79 cases of patients with breast tumor who were admitted from July 2015 to October 2018,with totally 96 BI-RADS 4 breast tumor.AR was calculated and compared with the pathological results.The subjects′work characteristic curve(ROC)was constructed based on the gold standard of pathological results to find the optimal cutoff point for the diagnosis of benign and malignant BI-RADS 4 breast tumors.Results According to the pathological diagnosis,54 of the 96 BI-RADS 4 breast tumors were benign and 42 were malignant.When AR was 1.095,it was the best diagnostic truncation point,with a specificity of 91.1%,a sensitivity of 74.5%,a diagnostic coincidence rate of 82.3%,and an area under ROC curve of 0.848.Conclusion AR has high clinical value in the diagnosis of BI-RADS 4 breast tumor.
