摘要
目的了解上海市青浦区金泽社区大肠癌发生情况及主要危险因素。方法 2012年5月起,在青浦区金泽社区开展老年人大肠癌筛查工作,筛查年龄段为男性60岁~74岁,女性50岁~74岁。筛查采用上海市疾病预防控制中心编制的大肠癌筛查危险度评估表和便隐血试剂,对阳性者开展纤维结肠镜诊断。结果在青浦区发现198名社区大肠癌患者,其中,金泽社区39例,dukes分期均为早中期。经单因素和多因素分析可知,性别、年龄、腌肉食用频率、体质指数(BMI)、慢性腹泻史、慢性便秘史、胆囊炎或者切除史、阑尾炎或者切除史、精神创伤为结肠癌发生的高危因素。结论社区大肠癌筛查便捷,可提高肿瘤的检出率和早发现率,有利于早期治疗和改善预后。
Objective To understand the occurrence of colorectal cancer and risk factors in Jinze Community of Qingpu District in Shanghai. Methods From May 2012, elderly screening for colorectal cancer were carried out at Jinze Community of Qingpu District. Screening age for males aged between 60-74 years old, women aged between 50 to 74 years old. The screening was performed using Colorectal Cancer Screening Risk Degree Assessment Scale designed by Shanghai Center for Disease Control and Prevention and Hemoccult, and colonoscopy was performed among the positive cases for diagnosis. Results A total of 198 community patients with colorectal cancer in Qingpu District were diagnosed, including 39 cases in Jinze Community with early Dukes stage. The univariate and multivariate analysis showed that colon cancer risk factors included gender, age, bacon eating frequency, body mass index, history of chronic diarrhea, chronic constipation history, cholecystitis or excision history, appendicitis or excision history and spiritual trauma. Conclusion The community colorectal cancer screening is convenient and sensitive, which can improve the detection rate of tumor, improve the early detection rate, and improve the early treatment and prognosis for community patients.
