摘要 目的 :通过比较自由排尿和经尿道尿管压力 流率测定的尿流参数 ,探讨尿管对尿流测定的影响。方法 :47例泌尿系症状患者 ,第 1组为 1 1例能在自由排尿时排尿而在压力 流率测定时不能排尿的患者 ,第 2组为 36例2次检查皆可排尿的患者 (包括 5例小儿患者 )。每个患者都进行了自由排尿 (无尿管 )和压力 流率测定 (有尿管 ) 2次检查。然后比较 2次检查的最大尿流率 (Qmax)、平均尿流率 (Qave)、残余尿 (PVR)、排出尿量 (VV)和达最大尿流时间 (Tmax)。结果 :1 1例成人 (2 3 % )患者带尿管时不能排尿。 31例 (66 % )成人患者 2次检查皆可排尿 ,2次尿流参数比较显示Qmax、Qave、Tmax和PVR差异有统计学意义 (P <0 .0 5)。VV差异无统计学意义 (P >0 .0 5)。在 5例小儿中 2次检查的 5项参数差异均无统计学意义 (P >0 .0 5)。结论 :在成年人尿管对尿流Qmax、Qave和PVF测定结果有影响。在小儿尿管对尿流测定影响较小。因此 ,对于成年人为了排除尿管对尿流测定的影响在进行压力 Aim: To investigate the impact of urethral catheter on uroflow in urodynamic studies by comparing the flow parameters of free voiding with those of voiding with transurethral catheter during the pressure flow study. Methods: The study included 47 patients who suffered from various urological or gynecological diseases. Group 1 included 11 patients who could void during free uroflowmetry but not during pressure flow study.Group 2 included 36 patients who could void during both tests.Each patient was required to perform pressure flow study (with urethral catheter) immediately after free uroflowmetry (with no urethral catheter). Five parameters including maximum flow rate (Qmax),average flow rate (Qave),post voiding residual volume (PVR),voided volume (VV) and time to Qmax (Tmax) were compared. Urodynamic studies were performed according to the recommendations proposed by International Continence Society (ICS). Results: Eleven patients (23%) could not void with catheter. For thirty one patients (66%) who could void the difference of Qmax,Qave,Tmax,and PVR was significant ( P <0.05), and VV was not significant between free and pressure flow study( P >0.05). For 5 children who could void in both tests, there was no significant difference in all the parameters ( P >0.05). Conclusion: The catheter has adverse impact on urine flow, especially in adult patients. However, the impact of catheterization on children are mild compared to adults. It is suggested that for adults, free uroflowmetry should be routinely carried out before the pressure flow study and the study should be interpreted combining with the free uroflowmetry.
