摘要
目的探讨COOK双球囊用于初产妇不同宫颈评分促宫颈成熟的临床对比效果。方法选取2014年8月至2017年12月在邢台市人民医院妇产科住院建档分娩的初产妇130例,根据Bishop评分情况分为对照组60例(Bishop评分1~5分)和观察组70例(Bishop评分≥5分),两组都给予COOK双球囊引产治疗,记录Bishop评分变化情况与引产效果。结果观察组与对照组治疗后的宫颈Bishop评分分别为(7.45±1.45)分和(7.34±1.82)分,明显高于治疗前的(5.82±0.47)分和(3.76±0.61)分,差异有统计学意义(P<0.05),治疗后两组宫颈Bishop评分比较,差异无统计学意义(P>0.05)。观察组与对照组的引产总有效率分别为97.1%和95.0%,两组引产总有效率比较,差异无统计学意义(P>0.05)。观察组第一产程时间明显短于对照组(P<0.05),第二、三产程时间比较,差异无统计学意义(P>0.05)。观察组新生儿窒息、产后出血、产后尿潴留等发生率比较,差异均无统计学意义(P>0.05)。结论 COOK双球囊用于不同宫颈评分的足月妊娠产妇都有很好的促宫颈成熟及引产效果,可显著改善宫颈条件,安全性较好,并可缩短产程。
Objective To investigate the clinical contrast effect of COOK double balloon for different cervical scores to promote cervical ripening.Methods A total of 130 primiparas who were delivered to Department of Obstetrics and Gynecology,Xingtai People’s Hospital from August 2014 to December 2017 were selected.According to the Bishop score,60 patients were in the observation group(Bishop score 1 to 5)and 70 cases(Bishop score≥5 points)in the control group,both groups were given COOK double balloon induction of labor,recording Bishop score changes and induction of labor.Results The cervical Bishop scores of the observation group and the control group were(7.45±1.45)points and(7.34±1.82)points,which were significantly higher than the(5.82±0.47)points and(3.76±0.61)points before treatment.The difference was statistically significant(P<0.05).There was no significant difference in the cervical Bishop scores between the two groups after treatment(P>0.05).The total effective rate of induction in the observation group and the control group were 97.1%and 95.0%,respectively.There was no significant difference in the total effective rate between the two groups(P>0.05).The first stage of labor in the observation group was significantly shorter than that in the control group(P<0.05),but there was no significant difference in the second and third stage of labor between the two groups(P>0.05).There was no significant difference in the incidence of neonatal asphyxia,postpartum hemorrhage and postpartum urinary retention in the observation group(P>0.05).Conclusion COOK double balloon is used for full-term pregnancy in different cervical scores.It has a good effect on cervical ripening and induction of labor,which can significantly improve cervical conditions,improve safety and shorten the labor process.
