心房颤动病人转律前房颤波谱的临床应用

摘要 目的 用房颤波谱分析技术 (AFW )对转律前的心房颤动病人进行研究 ,以确定其是否能够帮助临床选择可以成功转律的房颤患者。方法 将所有病人分为两组 ,A组为成功转律者 ,B组为不成功者 ,所有病人转律前均行AFW检查。结果 比较两组发现当房颤小波的峰频率明显减少 ,而对应的主导心房周长明显延长时 ,特别是F0max <3.5 2± 0 .16Hz ,MACL0 >32 5± 15 .0 1ms ,P4<3.62± 0 .33pw时转律的成功率较高 (P <0 .0 1)。结论 研究发现 ,心房扩大越明显、心房颤动持续的时间越长 ,AFW中除主要心房周长较短外 ,房颤小波各带的功率谱值均明显增加 ,转律的成功率低 。 Objective To evaluate efficiency of a spectra analysis system for atrial fibrillation waves(AFW)in selecting patients with defibrillation.Methods The patients were divided into two groups.The group A inclued patients with successful defibrillation,and group B included patients without.All patients had AFW test before defibrillation.Results ①The success rate of defibrillation was higher when the spike frequency was markedly decreased.The main atrial cycle length greatly increased.The most succesed Defibrillation was most successful when FOmax was less than 3.52±0.16Hz,MACL0 was more than 325±15.01ms and P4 was less than 3.62± 0.33pw(P<0.01).②There was a decreased successe rate of defibrillation when the MACL was shorter.Conclusion We have discovered that the longer the persistent time of atrial fibrillation,the shorter of the MACL.
