摘要
目的观察采用高粘度骨水泥椎体成形术配合增骨汤口服与保守疗法治疗腰椎压缩性骨折的临床疗效差异。方法将90例椎体压缩性骨折患者随机分为观察组和对照组,每组45例。观察组采用高粘度骨水泥椎体成形术治疗,术后配合增骨汤口服。对照组采用卧床休息,行腰背肌功能锻炼,配合增骨汤口服。分别于治疗前、治疗后1个月、3个月、12个月行腰椎正侧位X线检查,收集病椎的高度、Cobb角等数据,采用VAS、ODI评分标准进行功能评分,评价腰椎骨折的恢复情况。结果在椎体高度及Cobb角方面,在术后1个月、术后3个月时,两组间比较均无明显差异(P>0.05)。术后12个月,两组分别与自身的术后1个月相比,差异具有统计学意义(P<0.05)。在VAS评分方面,两组间比较无显著差异(P>0.05)。在ODI评分方面,两组间比较,术后1个月、3个月,观察组明显优于对照组(P<0.05)。结论对于无重大基础疾病的老年腰椎压缩性骨折的患者,高粘度骨水泥结合增骨汤治疗与保守疗法治疗,均能取得良好的临床疗效,在临床工作中,需要根据患者个体的具体情况来选择适合的治疗方案。
Objective To observe the curative differences of high-viscosity cemented vertebroplasty combined with Zenggu decoction and conservative treatment on lumbar vertebral compression fractures.Methods 90 cases of patients with vertebral compression fractures were randomly divided into observation group and control group,45 cases in each group.Observation group received high viscosity cementless vertebroplasty combined with Zenggu decoction.Control group received bed rest,back muscle functional exercise combined with Zenggu decoction.Positive and lateral X-ray examinations of lumbar spine were performed before treatment,1 month,3 months and 12 months after treatment.The height and Cobb angle of the diseased vertebrae were collected.VAS,ODI scores were used to evaluate the recovery of lumbar fracture.Results There was no significant difference in vertebral height and Cobb angle between the two groups at the 1st month and the 3rd month after operation.12 months after operation,there was a significant difference between the two groups and 1 month after operation(P<0.05).The difference between the two groups of VAS score was statistically analyzed at each time point with no statistical significance(P>0.05).The ODI scores of observation group were significantly better than those of control group at 1 and 3 months after operation(P<0.05).Conclusion For elderly patients with lumbar vertebral compression fracture without major underlying diseases,high-viscosity bone cement combined with Zenggu decoction and conservative treatment can achieve good clinical efficacy.In clinical work,it is necessary to select appropriate treatment options according to the specific conditions of individual patients.
