摘要
目的分析老年消化性溃疡(PUA)患者临床治疗以及内镜特点分析。方法研究样本选自本院2017年1月—2018年1月收治的49例老年消化性溃疡患者,另选择同期中青年消化性溃疡患者49例,对比两组患者治疗总有效率、预后生活质量、溃疡直径对比、并发症发生率等指标。结果老年组无症状、非特异症状和黑便、呕血为首发症状明显多于中青年组,而节律性上腹疼明显少于中青年组,差异有统计学意义(P <0. 05);老年组患者溃疡直径> 2. 0 cm明显多于中青年组,差异有统计学意义(P <0. 05);老年组胃溃疡和复合性溃疡发生率分别为53. 1%和16. 3%,明显高于中青年组,差异有统计学意义(P <0. 05);老年组患者并发症发生率明显高于中青年组,差异有统计学意义(P <0. 05)。结论老年消化性溃疡患者典型上腹痛少,无症状及非特异性症状多,其中胃溃疡发生率较高,在消化性溃疡患者治疗中,老年组患者相对而言治疗更为困难,且预后质量较低,容易引发并发症状,因此老年PUA要尽早采用胃镜做出正确诊断并采取相应治疗。
Objective To analyze the clinical treatment and endoscopic characteristics of elderly patients with peptic ulcer.Methods The study samples were selected from 49 elderly patients with peptic ulcer admitted to our hospital from January 2017 to January 2018,and 49 young and middle-aged patients with peptic ulcer in the same period were selected to compare the total therapeutic efficiency,prognosis quality of life,diameter of ulcer,complication incidence and other indicators.Results Asymptomatic,non-specific,black stool and hematemesis were the first symptoms of the aged group significantly more than the young and middle-aged group,while the rhythmic upper abdominal pain was significantly less than the young and middle-aged group,and the difference was statistically significant(P<0.05).The diameter of ulcer of the elderly group was significantly higher than that of the middle-aged group(>2.0 cm),and the difference was statistically significant(P<0.05).The incidence rate of gastric ulcer and complex ulcer in the elderly group was 53.1%and 16.3%,respectively,significantly higher than that in the young and middle-aged group,and the difference was statistically significant(P<0.05).The incidence of complications in the elderly group was significantly higher than that in the young and middle-aged group,and the difference was statistically significant(P<0.05).Conclusion Elderly patients with peptic ulcer on the typical abdominal pain less,no symptoms and nonspecific symptoms,high incidence of gastric ulcer,in treatment of patients with peptic ulcer,elderly group patients is relatively more difficult treatment,and prognosis of quality is low,easy to cause complication,so peptic ulcer will as soon as possible to adopt gastroscope to make the right diagnosis and take the corresponding treatment.
