摘要
目的:观察针刺疗法对脑梗死偏瘫患者下肢肌张力的影响。方法:将90例脑梗死偏瘫患者随机分为观察组和对照组,每组各45例。对照组行常规康复训练,观察组在对照组基础上联合针刺疗法治疗。比较两组患者治疗前后下肢痉挛情况及下肢运动功能评分、三维步态分析时空参数、时相参数及下肢关节活动范围。结果:两组患者治疗后MAS评分低于本组治疗前,FMA评分高于本组治疗前,且观察组MAS评分低于对照组,FMA评分高于对照组,差异均有统计学意义(P<0.05)。两组患者治疗后步速、步频、跨步长大于本组治疗前,步宽显短于本组治疗前,且观察组步速、步频、跨步长大于对照组,步宽短于对照组,差异均有统计学意义(P<0.05)。两组患者治疗后患侧支撑相百分比、患侧/健侧支撑相比值、双支撑相百分低于治疗前,患侧摆动相百分高于治疗前,且观察组患侧支撑相百分比、患侧/健侧支撑相比值、双支撑相百分比低于对照组,侧摆动相百分比高于对照组,差异均有统计学意义(P<0.05)。两组患者治疗后下肢关节活动度大于本组治疗前,且观察组大于对照组,差异均有统计学意义(P<0.05)。结论:针刺疗法有利于脑梗死偏瘫患者下肢肌张力恢复,从而改善肢体运动功能,改善偏瘫所致的步态异常。
Objective:To observe the influence of acupuncture on lower extremity muscle tension of patients with hemiplegia after cerebral infarction.Methods:Ninety patients with hemiplegia after cerebral infarction were randomly divided into the observation group and the control group,with 45 cases in each group.The control group received routine rehabilitation training,while the observation group received acupuncture treatment on the basis of the control group.The spasticity of lower limbs,motor function score of lower limbs,spatial and temporal parameters of three-dimensional gait analysis,temporal parameters and range of motion of lower limbs joints before and after the treatment were compared between the two groups.Results:After the treatment,MAS in each of the two groups was lower than that of the same group before the treatment,FMA was higher than that of each group before the treatment,and MAS in observation group was lower than that in control group,FMA was higher than that in control group,and all the differences were statistically significant(P<0.05).After the treatment,the step speed,step frequency and step length of the two groups were higher than those of the same group before the treatment,and the step width of the observation group was significantly lower than that before the treatment,and the step speed,step frequency and step length of the observation group were higher than those of the control group,and the step width of the observation group was lower than that of the control group,and all the differences were statistically significant(P<0.05).After the treatment,the percentage of supporting phase of the diseased side,the ratio of diseased side to healthy side and the percentage of double supporting phase were lower than those before the treatment.The percentage of swinging phase was higher than that before the treatment.The percentage of supporting phase of the diseased side,the ratio of diseased side to healthy side and the percentage of double supporting phase in observation group were lower than those
