摘要
目的探讨射频热凝靶点联合臭氧溶盘术治疗腰椎间盘突出症(lumbar disc herniation,LDH)髓核回缩效应的疗效。方法 LDH患者100例随机分为热溶组和单热组各50例,单热组给予射频热凝靶点治疗,热溶组在单热组治疗基础上给予臭氧溶盘术治疗。比较2组治疗前、后髓核最大前后径、横径和疼痛视觉模拟评分(visual analogue scale,VAS)、Oswestry功能障碍指数(Oswestry dysfunction index,ODI)评分及治疗优良率、并发症发生率。结果热溶组术前髓核最大前后径[(0.64±0.06)cm]、横径[(1.34±0.15)cm]、VAS评分[(6.06±1.89)分]、ODI评分[(57.92±10.08)分]与单热组[(0.65±0.06)cm、(1.32±0.14)cm、(5.94±1.83)分、(58.43±10.13)分]比较差异无统计学意义(P>0.05);热溶组术后髓核最大前后径[(0.40±0.04)cm]、横径[(1.01±0.10)cm]、VAS评分[(1.09±0.31)分]、ODI评分[(24.17±5.50)分]明显低于单热组[(0.47±0.05)cm、(1.10±0.12)cm、(2.64±0.68)分、(35.11±6.54)分](P<0.05),且2组均低于术前(P<0.05);热溶组治疗优良率(94%)明显高于单热组(82%)(P<0.05),并发症发生率(10%)与单热组(8%)比较差异无统计学意义(P>0.05)。结论射频热凝靶点联合臭氧溶盘术可有效改善LDH患者的髓核回缩效应、疼痛、功能,有利于提高患者的治疗疗效,且具有良好的安全性。
Objective To discuss the curative effect of radiofrequency thermocoagulation combined with ozone dissolution on the nucleus pulposus retraction in lumbar disc herniation(LDH).Methods Totally 100 patients with LDH were randomly divided into heat solution group and single heat group,with 50 patients in each group.Single heat group was treated with radiofrequency thermocoagulation,and heat solution group was treated with ozone dissolution besides radiofrequency thermocoagulation.The nucleus pulposus maximum anteroposterior diameter,transverse diameter,pain visual analogue scale(VAS)score,Oswestry dysfunction index(ODI),therapeutic effect and complication incidence were compared between two groups.Results There were no significant differences in the nucleus pulposus maximum anteroposterior diameter((0.64±0.06)cmvs.(0.65±0.06)cm),transverse diameter((1.34±0.15)cmvs.(1.32±0.14)cm),VAS score(6.06±1.89 vs.5.94±1.83)and ODI score(57.92±10.08 vs.58.43±10.13)between heat solution group and single heat group before operation(P>0.05).The postoperative nucleus pulposus maximum anteroposterior diameter,transverse diameter,VAS score and ODI score were significantly lower in heat solution group((0.40±0.04)cm,(1.01±0.10)cm,1.09±0.31,24.17±5.50)than those in single heat group((0.47±0.05)cm,(1.10±0.12)cm,2.64±0.68,35.11±6.54)(P<0.05),and were significantly lower than those before operation in both groups(P<0.05).The treatment excellent rate was significantly higher in heat solution group(94%)than that in single heat group(82%)(P<0.05),and there was no significant difference in the complication incidence between heat solution group(10%)and single heat group(8%)(P>0.05).Conclusion Radiofrequency thermocoagulation combined with ozone dissolution can effectively improve the nucleus pulposus retraction,relieve pain and improve the curative effect in patients with LDH,and it is safe.
