脊柱椎弓根螺钉内固定术后周围脓肿的双源CT图像质量研究

摘要 目的探讨脊柱椎弓根螺钉内固定术后周围脓肿患者的单能量、双能量CT图像质量差异。方法 27例脊柱椎弓根螺钉内固定术后周围脓肿患者,均行双源CT非增强扫描,扫描完成自动获得100keV和140keV及平均加权120keV图像。将100、140keV图像调入Syngo Dual Energy软件,经Monoenergetic程序处理得到40~190keV连续单能谱图像。选取50、80、110、140keV能谱值图像以及平均加权120keV图像进行多平面重建、最大密度投影及容积再现后处理,评价图像质量。结果平均加权120keV图像质量优良率为40.74%;50、80、110、140keV能量图像质量优良率分别为7.41%、81.48%、96.30%、55.56%;110keV能量图像质量优良率高于平均加权120keV图像及50、80、140keV能量图像,差异均有统计学意义(P<0.05)。结论双源CT双能量扫描技术中110keV能量图像可消除金属伪影,显示脓肿形态,在脊柱椎弓根螺钉内固定术后周围脓肿诊断中具有重要价值。 Objective To investigate the differences between single energy and dual-energy CT images of peripheral abscess after spinal pedicle screw fixation.Methods Twenty-seven patients with peripheral abscess after spinal pedicle screw fixation underwent non-enhanced dual-source CT scan to obtain 100,140 and averagely weighted 120 keV images.The 100 and 140keV images were transferred to Syngo Dual Energy software and processed by Monoenergetic program to get 40 to 190keV continuous single-energy images.The 50,80,110,140 and averagely weighted 120 keV images underwent multi-planar reconstruction,maximum intensity projection and volume rendering reconstruction to evaluate the image quality.Results The excellent and good rate of averagely weighted 120 keV image was 40.74%.The excellent and good rates of 50,80,110 and 140keV images were 7.41%,81.48%,96.30% and 55.56% respectively.The excellent and good rate of 110 keV image was significantly higher than 120,50,80 and 140keV images(P<0.05).Conclusion In dual-source CT dual-energy scanning technique,110 keV image can eliminate metal artifacts and show abscess morphology,and is of great value in the diagnosis of peripheral abscess after spinal pedicle screw fixation.
