后路椎弓根钉棒系统矫形内固定术应用自体髂骨移植对脊柱侧凸的效果及安全性分析 被引量:2

摘要 目的 :探讨后路椎弓根钉棒系统矫形内固定术在自体髂骨移植治疗脊柱侧凸的效果及安全性。方法 :选取108例脊柱侧凸患者为研究对象,根据移植骨材料不同分为自体组和异体组,自体组采用自体髂骨治疗,异体组采用同种异体骨治疗;观察两组患者术中情况,利用X射线测量患者Cobb角,对患者植骨融合情况进行评估。结果 :自体组手术时间及术中出血量均高于异体组(t分别为7.840、9.057);自体组引流管放置时间、体温、白细胞计数、血沉恢复正常时间低于异体组(t分别为33.424、3.147、13.766、27.341);两组Cobb角丢失情况及植骨融合率无明显差异,两组植骨融合率比较差异无统计学意义;自体组7例患者出现骨区疼痛,异体组有2例患者出现植骨部位活性感染。结论 :自体髂骨治疗脊柱侧凸,具有显著的临床疗效,但患者易发生取骨处疼痛,在治疗过程中应给予相关的对症处理。 Objective To evaluate the efficacy and safety of posterior pedicle screw rod system internal fixation in the treatment of scoliosis with autogenous iliac bone graft.Methods 108 cases of scoliosis were selected as subjict and divided into autologous and allogeneic groups according to the different bone graft materials,autologous iliac bone was used in the autologous group,and allograft was used to treat the allograft;the intraoperative condition of the two groups was observed,and the Cobb angle was measured by X ray.Results The operative time and intraoperative blood loss in the autologous group were higher than those in the allograft group(t=7.840,9.057);the time of placement,body temperature,white blood cell count and erythrocyte sedimentation rate in autologous group were lower than that of the allogeneic group(t was 33.424,3.147,13.766,27.341);there was no significant difference in Cobb angle loss and bone graft fusion rate between the two groups.There was no significant difference in bone graft fusion rate between the two groups;in the autologous group,bone pain occurred in 7 patients,and in 2 patients in the allograft group,there was active infection at the bone graft site.Conclusion The treatment of scoliosis with autogenous iliac bone has significant clinical effects,but patients are prone to pain at the place where the bone is removed.Symptomatic treatment should be given during the treatment.
