冠心宁对脑梗死患者血清心型脂肪酸结合蛋白、神经元特异性烯醇化酶水平及颈动脉粥样斑块硬化的影响 被引量:4

Effects of Guanxinning Injection on Serum Heart-type Fatty Acid Binding Protein,Neuronspecific Enolase and Carotid Atherosclerotic Plaque in Cerebral Infarction Patients

摘要 目的:探讨冠心宁对脑梗死患者血清心行脂肪酸结合蛋白、神经元特异性烯醇化酶水平及颈动脉粥样硬化的影响。方法:选择我院神经内科收治的260例患者为研究对象,按照住院先后顺序随机分为对照组与观察组,每组各130例,患者入院后均予以脱水降颅压、调节血糖、控制血压、溶栓、营养脑神经等综合治疗,观察组在综合治疗上予以中药剂冠心宁注射液,15天为一个疗程,共治疗1个疗程。疗程结束后,观察两组神经功能改善情况,并检测治疗前后血清心型脂肪酸结合蛋白(H-FABP)、神经元特异性烯醇化酶(NSE)水平,并使用颈动脉超声检查两组颈动脉粥样硬化及脑血管储备功能情况;并检测两组血栓相关因子及血管内功能,采用NIHSS评分比较神经功能变化,统计不良反应。结果:治疗后对照组总有效率82.3%,显著低于观察组90.7%;治疗后对照组H-FABP、NSE、CVR依次为(308.63±31.29)pg/m L、(25.8±3.6)ng/m L、(5.78±0.75),显著高于观察组(295.47±34.52)pg/m L、(12.94±1.52)ng/m L、(28.6±4.6),差异具有统计学意义;治疗后对照组IMT、内膜斑块面积依次为(1.31±0.14)mm、(0.64±0.07)cm2,易损斑块56.9%,均显著高于观察组(1.12±0.13)mm、(0.52±0.06)cm2、37.7%;治疗后两组血栓相关因子D-二聚体、PAg T及血管内皮因子ET-1水平显著低于治疗前,NO显著高于治疗前,且治疗后观察组各项指标变化幅度均显著高于对照组;对照组不良反应总发生率17.7%,与观察组13.07%比较,差异无统计学意义。结论:冠心宁通过增加梗死区血供保护脑神经细胞,减少脑损伤发生,并减轻血管炎症,调节机体代谢,减轻颈动脉粥样硬化状态,减少不可逆性神经损伤,且未增加不良反应,适于治疗各种脑血管疾病。 Objective To explore the effects of Guanxinning injection on serum heart-type fatty acid binding protein, neuron-specific enolase level and carotid atherosclerosis in patients with cerebral infarction. Methods 260 cases of patients treated in the department of cardiovascular medicine of our hospital were selected for the study and randomly divided into the control group and the observation group with 130 cases in each group according to the order of admission. All the patients were given dehydration reducing intracranial pressure, blood glucose regulating, blood pressure control, thrombolysis, brain nutrition and other comprehensive treatment after admission, and the observation group was given Chinese herbal medicine of Guanxinning injection on this basis of treatment.15 days was for a course of treatment, and the two groups were treated for 1 course. After the course, the improvement of neurological function was observed between the two groups, and the levels of serum hearttype fatty acid binding protein(H-FABP), neuron-specific enolase(NSE) were measured before and after treatment. The carotid artery atherosclerosis and cerebrovascular reserve function were examined by carotid ultrasonography. The thrombosisrelated factors and intravascular function were detected between the two groups. And the NIHSS score was used to compare the changes of neurological function and the adverse reactions were counted. Results The total effective rate was 82.3% in the control group, which was significantly lower than that in the observation group with 90.7%. The levels of H-FABP, NSE and CVR after treatment were(308.63±31.29) pg/m L,(25.8±3.6) ng/m L and(5.78±0.75) respectively in the control group, which were significantly higher than those in the observation group with(295.47±34.52) pg/m L,(12.94±1.52) ng/m L and(28.6±4.6).
