摘要
目的:探讨非布司他与别嘌醇治疗痛风合并肾功能不全的临床疗效。方法:选择98例痛风合并肾功能不全的患者为研究对象,根据随机数字表法将患者随机分为观察组(n=49)与对照组(n=49),对照组予别嘌醇治疗,观察组则予非布司他治疗,比较两组患者治疗前后血尿酸水平及估算的肾小球滤过率(e GFR)的变化,记录药物不良反应。结果:两组患者治疗后,血尿酸水平均较治疗前明显降低,差异有统计学意义;但是在治疗后4周、8周及12周时,观察组患者血尿酸水平均明显低于对照组,差异有统计学意义;治疗后,观察组患者血尿酸达标率明显高于对照组,不良反应发生率明显低于对照组,差异均有统计学意义;观察组治疗后e GFR较治疗前明显上升,且高于对照组,差异有统计学意义。结论:非布司他治疗痛风合并肾功能不全可有效降低血尿酸水平,有助于肾功能的保护,且具有较好的用药安全性。
Objective To investigate the clinical effect of febuxostat and allopurinol in treatment of patients with gout and renal insufficiency. Methods 98 cases of patients with gout and renal insufficiency as the research object, according to the random number table method, the patients were randomly divided into observation group(n=49) and control group(n=49), the control group was treated with allopurinol treatment, the observation group was treated with febuxostat treatment, serum uric acid level and estimated glomerular filtration rates(e GFR) were compared between two groups before and after treatment, the adverse drug reaction was recorded. Results The serum uric acid levels in the two groups after treatment, were significantly lower than that before treatment, the difference was of statistically significant; but in 4 week after treatment, and 8 week and 12 week, the blood uric acid level in the observation group were significantly lower than the control group, the difference was of statistically significant; the blood uric acid compliance rate in the observation group was significantly higher than the control group, and the adverse reaction rate was significantly lower, the difference was of statistically significant; the e GFR in observation group after treatment was increased significantly than before treatment, higher than the control group, the difference was of statistically significant. Conclusion The treatment of febuxostat gout and renal insufficiency can effectively reduce the level of serum uric acid, is helpful for the protection of renal function, and has good safety.
