摘要
目的 :分析并评价主动固定电极应用于心房起搏的临床效果和安全性,同时对右房高位房间隔、右房低位房间隔和右心耳等部位的起搏器安置疗效进行比较。方法 :依据纳入排除标准共选取90例需要安置永久性心脏起搏器的患者并随机分为2组,对照组采用被动固定电极,研究组采用主动固定电极,根据起搏部位又可分为右房高位房间隔、右房低位房间隔和右心耳;比较患者术后即刻、1周、1个月心房起搏电极的基本参数(阻抗、起搏阈值、P波幅度)以及客观不良情况(心率失常、冠心病、高血压、心力衰竭)。结果 :研究组患者心房阻抗、起搏阈值、P波幅度等起搏参数较对照组有明显差异,均有统计学意义;研究组不良情况发生率13.33%显著低于对照组40.00%,有统计学差异。各部位的起搏器安置疗效具有一定的差异性,高位房间隔明显优于低位房间隔和右心房,有统计学差异。结论 :针对心率失常的患者在右房高位间隔安置主动固定电极,起搏的临床效果较佳,既可有效治疗患者的疾病,又可避免重型并发症的发生,显著提高治疗有效率,值得临床推广使用。
Objective To analyze and evaluate the clinical effect and security of active fixation lead on atrium pulse. Method Selected 90 patients who needed to install active fixation lead according to inclusive and exclusive criteria. They were divided into two groups randomly, control group used passive fixation lead, research group used active fixation lead; According to the pacing site can be divided into high right atrial septal, low right atrial septal, high right atrial septal and right atrial appendage;compared patients’ atrial pacemaker electrode basic parameters(impedance, pacing threshold, P wave amplitude) and adverse reactions(arrhythmia, coronary heart disease, high blood pressure) postoperative immediately, a week and a month Result Research group’s impedance, pacing threshold, P wave amplitude had obvious difference compared with control group, with statistical significance; Research group’s adverse reaction rate 13.33% was obvious lower that 40.00% of control group, with statistical difference. Conclusion Active fixation lead has good effect on atrium pulse, it can treat patients’ illness effectively and can avoid serious complications, it is worthy of promotion in clinical because it can improve effective rate of treatment.
