摘要
目的比较甘油三酯葡萄糖乘积指数(TyG index)与甘油三酯高密度脂蛋白胆固醇比值(TG/HDL-C对代谢综合征(MS)的评价作用,选择筛查MS的简易指标。方法选取2015年1～12月于北京市房山区第一医院门诊体检人员349例(年龄20～66)岁,分别进行询问病史、体格检查、生化检查,并计算Ty G index、TG/HDL-C。依据中华医学会糖尿病学分会(CDS)诊断标准诊断MS患者125例。对MS组与非MS组受试者的TyG index、TG/HDL-C数据进行比较。采用SPSS18.0统计软件绘制TyG index、TG/HDL-C评价MS受试者的工作特征(ROC)曲线,计算曲线下面积(AUC),并采用MedCalc16.8软件对AUC进行分析。结果 349例受试者中男性134例,MS 51例;女性215例,MS 74例;MS组患者的TyG index和TG/HDL-C均值分别为(9.3±0.6)、(2.4±2.1),均明显高于非MS组的(8.5±0.5)和(1.2±1.2),差异均有统计学意义(P<0.05);男性TyG index、TG/HDL-C评价MS的AUC分别为0.88、0.81,二者比较差异有统计学意义(P<0.05);女性TyG index、TG/HDL-C评价MS的AUC分别为0.85、0.83,两者比较差异无统计学意义(P>0.05);总体Ty G index、TG/HDL-C评价MS的AUC分别为0.86、0.82,二者比较差异有统计学意义(P<0.05);Ty G index评价MS风险切点,男性8.83,女性8.72,总体8.80;TG/HDL-C评价MS风险切点,男性1.48,女性1.21,总体1.21。结论 TyG index及TG/HDL-C均与MS密切相关;TyG index在男性及总体人群中对MS风险的评价作用优于TG/HDL-C,适于作为MS的筛查指标。
Objective To compare the function of triglyceride-glucose product(TyG index)and triglyceride-to-high density lipoprotein cholesterol ratio(TG/HDL-C)in the evaluation of metabolic syndrome(MS),and to determine the screening indicator of MS.Methods A total of 349 subjects(aged 20 to 66)who had a medical examination at Beijing Fangshan First Hospital during Jan.2015 to Dec.2015 were included in this analysis.The subjects were asked about their medical history,and all underwent physical examination and biochemical examination.The value of TyG index and TG/HDL-C were calculated.A total of 125 patients were diagnosed as MS according to the Chinese Diabetes Society(CDS)criteria.Ty G index and TG/HDL-C mean values were compared between MS patients and non-MS patients.SPSS18.0 was used to draw the ROC curve of Ty G index and TG/HDL-C for the diagnosis MS and to calculate the area under the curve(AUC).The AUC was compared and analyzed with Med Calc16.8 software.Results Among the 349 subjects,there were 134 males(including 51 MS patients)and 215 females(including 74 MS patients).The mean Ty G index,and TG/HDL-C of MS patients were 9.3±0.6,and 2.4±2.1,significantly higher than 8.5±0.5,and 1.2±1.2 of non-MS patients(P<0.05).AUC values of Ty G index,and TG/HDL-C for evaluating MS of male patients were 0.88,and0.81,respectively,and there was a statistically significant difference(P<0.05).And the values for evaluating MS of female patients were 0.85,and 0.83,respectively,with no statistically significant difference(P>0.05).Those for evaluating MS of all subjects were 0.86,and 0.82 respectively,and there was a statistically significant difference(P<0.05).The cut-off points of Ty G index for evaluating MS were 8.83(males),8.72(females),and 8.80(all subjects),and the cut-off points of TG/HDL-C for evaluating MS were 1.48(males),1.21(females),and 1.21(all subjects).Conclusion TyG index and TG/HDL-C were closely related to the MS.TyG index is superior to TG/HDL-C in the evaluation of MS in both males and total subjects.It is more
