摘要
目的研究血清半乳甘露聚糖(GM)联合血浆1,3-β-D葡聚糖(BG)检测对侵袭性真菌病(IFD)的临床诊断价值。方法选取我院于2013年3月至2015年12月期间收治的高危IFD患者252例,检测和分析其GM及BG浓度,评价GM、BG单独和联合检测对IFD的诊断效能。结果 252例患者中,GM检测阳性68例(26.98%),BG检测阳性83例(32.94%),GM+BG检测阳性68例(26.98%);GM检测对IFD诊断的特异度、灵敏度、阴性及阳性预测值依次为97.42%、65.98%、82.07%、94.12%,BG检测依次为82.85%、69.07%、82.85%、80.72%,GM+BG依次为99.35%、69.07%、83.69%、98.53%,GM+BG的特异度、阴性及阳性预测值均明显高于BG检测,差异有统计学意义(P<0.05);GM+BG检测的特异度、阴性及阳性预测值均略高于GM检测,但差异无统计学意义(P>0.05)。结论 GM联合BG检测有利于提高IFD的诊断效能,相较于单一指标检测具有更高的特异性及阳性预测作用,对IFD诊断及治疗具有重要的辅助意义。
Objective To study the clinical diagnostic value of serum galactomannan(GM) combined with plasma(1,3)-β-D-glucan(BG) on invasive fungal disease(IFD). Methods A total of 252 patients with high risk IFD in our hospital from March 2013 to December 2015 were selected, in which GM and BG concentrations were detected and analyzed. The diagnostic efficacy of GM, BG and combined detection on IFD were evaluated. Results Among 252 patients, GM test were positive in 68 cases(26.98%), and BG test were positive in 83 cases(32.94%). GM+BG test were positive in 68 cases(26.98%). The specificity, sensitivity, negative and positive predictive value in the diagnosis of IFD were respectively 97.42%, 65.98%, 82.07%, 94.12% for GM detection, 89.68%, 69.07%, 82.85%, 80.72% for BG detection, and 99.35%, 69.07%, 83.69%, 98.53% for GM+BG detection. The specificity, negative and positive predictive valueof GM+BG detection were significantly higher than those of BG detection, and the differences were statistically significant(P<0.05). The specificity, negative and positive predictive value of GM+BG detection were slightly higher than those of GM detection, but the difference was not statistically significant(P>0.05). Conclusion GM combined with BG detection is helpful to improve the diagnostic efficacy of IFD, which has a higher specificity and positive predictive effect compared with single index detection. It has important auxiliary significance for the diagnosis and treatment of IFD.
