摘要
目的观察内镜下胃造瘘术胃肠内营养支持对危重患者营养状况的改善效果。方法将2012年12月至2014年12月在我院及协作单位接受治疗的160例危重症患者纳入研究,按随机数表法分为观察组79例和对照组81例,分别接受内镜下胃造瘘术胃肠内营养支持和鼻胃管鼻饲胃肠内营养支持,连续治疗2周后,比较两组患者的体质量指数(BMI)、白细胞、血红蛋白、血清前白蛋白、白蛋白、淋巴细胞计数、CD3+、CD4+、CD8+,以及并发症等指标。结果治疗后观察组的BMI、血红细胞、血清前白蛋白、白蛋白高于对照组[(19.33±1.84)kg/m2vs(18.67±1.56)kg/m2;(99.5±12.4)个/L vs(92.4±12.7)个/L;(246.2±26.3)g/L vs(225.7±29.9)g/L;(38.7±4.1)g/L vs(36.1±3.8)g/L],差异均有统计学意义(P<0.05),而白细胞、CD8+明显低于对照组[(7.32±1.32)个/L vs(8.45±1.84)个/L;(24.84±4.08)个/μL vs(28.44±4.07)个/μL],差异均有统计学意义(P<0.05);在淋巴细胞、CD3+、CD4+方面,观察组明显高于对照组[(2.92±0.57)个/L vs(2.32±0.62)个/L;(67.84±8.33)个/μL vs(62.17±5.48)个/μL;(42.36±6.11)个/μL vs(36.18±4.92)个/μL],差异均有统计学意义(P<0.05);观察组并发症发生率为20.3%(16/79),明显低于对照组的50.6%(41/81),差异具有统计学意义(P<0.05)。结论与传统鼻饲管胃肠内营养支持方式相比,内镜下胃造瘘术胃肠内营养支持对改善危重患者营养状况的有效性和安全性更为显著,值得临床推广应用。
Objective To discuss the effect of enteral nutrition support of endoscopic gastrostomy on improving nutritional status, and provide theoretical basis for clinical work. Methods A total of 160 cases of critically ill patients hospitalized from December 2012 to December 2014 in our department,were selected as the study objects. According to the random number table, these patients were divided into experimental group(n=79) and control group(n=81),and respectively received enteral nutrition support of endoscopic gastrostomy and nasogastric tube intestinal nutrition support. After continuous treatment for two weeks, body mass index(BMI), white blood cell, hemoglobin, serum albumin, albumin, lymphocyte count, CD3+,CD4+,CD8+and complication indexes of each group were observed and compared. Results After the treatment, BMI, red blood cell, serum pre albumin, albumin in the experimental group [(19.33±1.84) kg/m2,(99.5±12.4)/L,(246.2±26.3) g/L,(38.7±4.1) g/L, respectively] were significantly higher than those in the control group [(18.67±1.56 kg/m2,(92.4±12.7) /L,(225.7±29.9) g/L,(36.1±3.8) g/L, respectively](P<0.05). However,white blood cells and CD8+in the experimental group [(7.32 ± 1.32) /L and(24.84 ± 4.08) /μ L] were significantly lower than those in the control group [(8.45±1.84) /L and(28.44±4.07) /μL](P<0.05). Lymphocyte, CD3+and CD4+in the experimental group [(2.92±0.57)/L,(67.84±8.33)/μL,(42.36±6.11)/μL, respectively] were significantly higher than those in the control group [(2.32±0.62)/L,(62.17±5.48) /μL,(36.18±4.92)/μL, respectively](P<0.05). The incidence rate of complications in the observation group and control group were respectively 20.3%(16/79) and 50.6%(41/81), and the difference was statistically significant(P<0.05).
