摘要
目的研究食管癌患者的癌组织发生淋巴结转移与病变组织中血管内皮生长因子(VEGF)、细胞的粘附因子C4D4的变异体v6(CD44v6)、基质金属蛋白酶-2(MMP-2)水平的关系。方法选取2011年3月至2014年9月到我院肿瘤科进行治疗的83例食管癌患者作为食管癌组,所有患者均经术后HE切片病理检查证实,另选取21例术前诊断为食管癌但术后经组织学检查结果为良性者的食管黏膜标本作为良性组,比较两组患者中VEGF、CD44v6和MMP-2的阳性水平,并对不同表达食管癌患者的淋巴结是否发生转移进行分析,探讨他们的相关性。结果 VEGF、CD44v6和MMP-2在食管癌组患者病变组织标本中的阳性表达率分别为77.11%(64/83)、73.49%(61/83)和59.04%(49/83),而在良性组患者食管组织标本中的阳性表达率分别为14.29%(3/21)、23.81%(5/21)和14.29%(3/21),三者在食管癌组患者病变组织标本中的阳性表达率均显著高于良性组,经比较差异均有统计学意义(P<0.05);VEGF、CD44v6和MMP-2在发生了淋巴结转移的食管癌患者中的阳性率依次为85.48%(53/62)、82.26%(51/62)和66.13%(41/62),而在没有发生淋巴结转移的患者中的阳性率依次为52.38%(11/21)、47.62%(10/21)和38.10%(8/21),发生淋巴结转移的食管癌患者病变组织标本中三者的阳性表达率显著高于没有发生转移者,经比较差异均具有统计学意义(P<0.05)。结论 VEGF、CD44v6和MMP-2在食管癌患者癌变组织中的表达水平与其淋巴结是否发生转移具有一定的关系,其对食管癌患者的早期正确诊断、治疗以及预后效果评估具有重要的意义。
Objective To study the correlation between vascular endothelial growth factor(VEGF), CD44 variant 6(CD44v6), matrix metalloproteinase-2(MMP-2) level and lymph node metastasis in patients with esophagus cancer. Methods From March 2011 to September 2014, 83 patients of esophageal cancer treated in our hospital and confirmed by postoperative pathological examination were selected as esophageal cancer group, and 21 patients diagnosed as esophageal cancer preoperatively but confirmed as benign by postoperative pathological examination were enrolled as the benign group. The positive expression levels of VEGF, CD44v6 and MMP-2 were compared between the two groups, and the lymph node metastasis in patients with different expression levels was analyzed to explore their correlation. Results The positive expression rate of VEGF, CD44v6 and MMP-2 in esophageal cancer group were 77.11%(64/83), 73.49%(61/83) and 59.04%(49/83), respectively, which were significantly higher than those in the benign group of 14.29%(3/21), 23.81%(5/21) and 14.29%(3/21)(P<0.05). The positive expression rate of VEGF, CD44v6 and MMP-2 in patients with lymph node metastasis were 85.48%(53/62), 82.26%(51/62) and 66.13%(41/62), which were significantly higher than those in the patients without lymph node metastasis of 52.38%(11/21), 47.62%(10/21) and38.10%(8/21)(P<0.05). Conclusion The expression levels of VEGF, CD44v6 and MMP-2 in esophageal cancer are correlated to lymph node or metastasis, which has important significance in the early diagnosis, treatment, and prognosis assessment of esophageal cancer.
