摘要
目的:探讨喉罩在腹腔镜成人腹股沟疝修补术全身麻醉中的应用效果。方法:选择2016年4月至2018年5月本院普外科收治的拟择期行腹腔镜成人腹股沟疝修补术的患者80例,按入院顺序分为对照组(n=40)和观察组(n=40),分别采用气管插管全麻及喉罩全麻。比较两组术中呼吸循环指标、手术及苏醒情况、并发症发生情况。结果:观察组术中平均动脉压(MAP)变异指数、心率变异指数、最高呼气末二氧化碳分压(PETCO2)和麻醉苏醒时间均低于对照组,术中最低脉搏血氧饱和度(Sp O2)高于对照组(均P<0.05)。两组术中失血量、手术时间差异无统计学意义(均P>0.05)。观察组与对照组并发症发生率分别为20.0%(8/40)、50.0%(20/40),观察组显著低于对照组(P<0.05)。结论:腹腔镜成人腹股沟疝修补术全身麻醉中的应用喉罩,可稳定呼吸循环,降低并发症发生风险。
Objective:To investigate the efficacy of using laryngeal mask in general anesthesia for laparoscopic repair of inguinal hernia in adults.Methods:Eighty adult patients who underwent elective laparoscopic repair of inguinal hernia in our Department of General Surgery between April 2016 to May 2018 were enrolled and divided into the control group(n=40)and the study group(n=40)according to the time of hospital admission,to receive endotracheal intubation or laryngeal mask in general anesthesia,respectively.The two groups were compared for intraoperative respiratory and circulatory indices,surgical procedure,recovery,and complications.Results:The mean arterial pressure(MAP)variability,heart rate variability,highest endtidal carbon dioxide partial pressure(PETCO2)and time to recovery from anesthesia were lower or shorter in the study group than those in the control group;the lowest pulse oxygen saturation(Sp O2)during the operation was higher in the study group than that in the control group(all P<0.05).There were no significant differences in blood loss and operation time between the two groups(P>0.05).The incidence rate of complications in the study group was 20.0%(8/40),which was significantly lower that in the control group[50.0%(20/40)](P<0.05).Conclusion:The use of laryngeal mask in general anesthesia for laparoscopic repair of inguinal hernia in adults may help stabilize the respiratory and circulatory conditions as well as reduce the risk of complications.
