摘要
目的观察益心通络胶囊防治冠心病支架置入术后微循环障碍的作用机制。方法将诊断明确的冠心病PCI术后微循环障碍患者80例,随机分为治疗组40例和对照组40例,对照组给予西医常规治疗,治疗组在对照组基础上加用益心通络胶囊。2组均治疗8周,比较2组治疗前后微循环阻力系数(IMR)、慢血流现象(CSFP)、内皮细胞损伤因子(ET-1)、白细胞介素6(IL-6)、超敏C反应蛋白(hs-CRP)和中医症状积分的变化。结果治疗后,治疗组中医症状总疗效优于对照组(P <0. 05);同治疗前比较,2组均能改善中医症状积分,治疗组积分改善程度优于对照组(P <0. 01);治疗后2组微循环阻力系数均较治疗前下降,治疗组优于对照组(P <0. 01);在TIMI血流分级方面,治疗组TIMI血流分级改善程度优于对照组(P <0. 05);2组治疗后均能降低ET-1、IL-6、hs-CRP水平,治疗组在改善内皮损伤,抑制炎症因子方面优于对照组(P <0. 05或P <0. 01)。结论益心通络胶囊能降低中医症状积分,抑制炎症反应,纠正内皮细胞损伤,改善冠心病支架置入术后微循环障碍,提高临床疗效,为中医学在治疗冠脉微循环障碍方面提供重要临床依据。
Objective To observe the mechanism of Yixin Tongluo capsule in preventing microcirculation disturbance after coronary artery disease stent implantation.Methods 80 patients with microcirculatory disturbance after PCI were randomly divided into treatment group(40 cases)and control group(40 cases).The control group was treated with routine western medicine,and the treatment group was treated with Yixin Tongluo capsule on the basis of the control group.The changes of microcirculatory resistance coefficient(IMR),slow flow phenomena(CSFP),endothelial cell injury factor(ET-1),interleukin-6(IL-6),hypersensitive C-reactive protein(hs-CRP)and symptom score of TCM before and after treatment were compared between the two groups.Result After treatment,the total curative effect of TCM symptoms in the treatment group was better than that in the control group(P<0.05).Compared with before treatment,both groups could improve TCM symptom scores,and the improvement degree of TCM symptom scores in the treatment group was better than that in the control group(P<0.01).After treatment,the resistance coefficient of microcirculation in both groups decreased compared with that before treatment,and the treatment group was better than the control group(P<0.01).In terms of TIMI blood flow grading,the improvement of TIMI blood flow grading in the treatment group was better than that in the control group(P<0.05).Both groups could reduce the levels of ET-1,IL-6 and hs-CRP after treatment,and the treatment group was better than the control group in improving endothelial injury and inhibiting inflammatory factors(P<0.05 or P<0.01).Conclusion Yixin Tongluo capsule can reduce TCM symptom score,inhibit inflammation reaction,correct endothelial cell injury,improve microcirculation disturbance after coronary artery disease stent implantation and clinical therapeutic effect,and provide important clinical basis for TCM in the treatment of coronary microcirculation disturbance.
