摘要
目的 了解糖皮质激素对系统性红斑狼疮 (SLE)患者骨代谢水平的影响。方法 用放射免疫分析法对 32例未经糖皮质激素治疗和 38例经糖皮质激素治疗的SLE患者的血清骨钙素 (BGP)和尿脱氧吡啶啉(DPD)进行测定 ,血Ca2 + 、P及AKP进行测定 ,并与 30名健康人进行对照。结果 未经糖皮质激素治疗组血清Ca2 + 、BGP水平较正常对照组明显下降 (P <0 0 5 ) ;尿DPD水平较正常对照组明显升高 (P <0 0 5 ) ;使用激素治疗后的SLE患者血清BGP水平较未使用组明显下降 (P <0 0 5 ) ,尿DPD水平明显升高 (P <0 0 5 )。结论 SLE患者本身就存在骨形成障碍和骨吸收增加 。
Objective To understand the influence of glucocorticoid to bone metabolism of patients with systemic lupus erythematosus (SLE).Methods Seventy patients with SLE and 30 matched normal controls were studied,the serum Ca 2+ ,P,AKP osteocalcin level and urine deoxypyridinoline level were measured by RIA.Results The serum osteocalcin level of SLE patients without glucocorticoid therapy was significantly lower than that of the normal subjects ( P <0 05),the serum osteocalcin level of SLE patients with glucocorticoid therapy was significantly lower than that of those without glucocorticoid therapy ( P <0 05),but the urine deoxypyridinoline level of SLE patients with glucocorticoid therapy significantly increased ( P <0 05).Conclusion Abnormal bone formation and decreased bone turnover exist in SLE patients;when the SLE patients receive glucocorticoid therapy,their abnormal bone metabolism becomes even more serious.
