横向连接装置在腰椎经椎弓根内固定系统中的作用 被引量:8

摘要 目的 :评价横向连接装置在腰椎后路内固定器械中的作用 ,进而为临床应用提供生物力学依据。方法 :应用 6具新鲜腰骶椎尸体标本 ,在三维运动实验机上模拟人体脊柱活动 ,分别测定腰椎不稳、予以短节段经椎弓根CD器械固定 (应用或不应用横向连接杆 )不同状态下L4~ 5节段运动范围 (ROM)。随后将安装CD后的标本试件在 858MiniBionix -MTS多轴实验机上模拟人体生理活动行 1 50 0次疲劳运动 ,并测定相应节段的ROM。结果 :①安装CD内固定后 ,应用横向连接杆 (DTT)组与不应用横向连接杆(NDTT)组在前屈 /后伸、左 /右侧弯位时L4~ 5节段ROM小于完整结构组 ,但轴向旋转时无DTT组ROM大于完整结构组 ;②疲劳试验后DTT组与无DTT组ROM均增加 ,而DTT组在各方向的ROM值均明显小于NDTT组。结论 :横向连接装置可明显增强失稳脊椎内固定的作用 。 Objective: To evaluate the biomechanical effect of cross-link device (CLD) in transpedicle fixation systems in order to provide the better understanding for the clinical application. Methods: A total of six fresh human cadaveric specimens (L 2-S 3) were mounted to the Spinal 3D Test Machine which could imitate the three-dimensional motions of the spine under various conditions of the intact spine, L 4~5 segment instability and CD internal fixations (DTT or NDTT). The range of motion (ROM) was determined by Stereophotogrammetry System. In addition, the specimens mounted to 858Mini Bionix-MTS were loaded 1500-cycle-fatigue motions so as to the physiological condition can be simulated, and then the ROM of L 4~5 segment were measured. Results: ① Stabilized by CD, both the situations of DTT and NDTT had a smaller ROM than that of intact lumbar while performing flexion/extension and lateral bending. But NDTT had a bigger ROM in axial rotation. ② After fatigue loading, a marked increase of ROM could be seen in DTT and NDTT which were even bigger than that in intact spines. But DTT still had a smaller ROM in the three-dimensional orientations compared with NDTT. Conclusion: CLD can significantly increase the stiffness of internal fixations and made the lumbar more stable.
