摘要
小陷胸汤出自《伤寒论》,是经典名方之一。采用溯源法和文献计量学的方法,对小陷胸汤的历史源流、组成、剂量、功效、炮制、主治病证病位进行考证分析,并归纳其古今临床应用。共检索189条相关古籍文献,涉及85本古籍,现代文献515条。考证结果表明,小陷胸汤由“黄连一两,半夏半升(洗),栝蒌实大一枚”组成,现代常用剂量为瓜蒌15~40 g,黄连5 g,半夏12 g。小陷胸汤原治疗小结胸病,全方清热化痰,宽胸散结,现代临床应用不囿于心下之病位,在古籍记载基础上有所延伸与扩展,广泛应用于治疗胸膜炎、急性支气管炎、肺气肿、心绞痛、急慢性胃炎、食管炎、胆囊炎等多种疾病。该方的使用注意主要有以下几点:(1)剂量,黄连用量不宜过大,防苦寒降泻之品伤阳;(2)半夏炮制,建议用矾制,解毒保效的同时易于保存;(3)瓜蒌实入药并先煎,全果实入药指瓜蒌实连皮带仁,可加强散结的作用;(4)辨治要点为舌红苔黄腻,脉滑数,明病机为痰热互结即可应用该方。
Xiaoxianxiong Decoction comes from the Treatise on Febrile Diseases,and is one of the classic formulas.By using the methods of traceability and bibliometrics,the historical origin,composition,dosage,efficacy,concoctions,and primary treatment of Xiaoxianxiong Decoction were examined and analyzed,and its ancient and modern clinical uses were summarized.A total of 189 related ancient documents were searched,involving85 ancient books and 515 modern documents.The research results showed that Xiaoxianxiong Decoction is composed of "one pieces of Huanglian(Coptidis Rhizoma),half a liter of Banxia(Pinelliae Rhizoma)(washed),and one large piece of Gualou(Trichosanthes)",The commonly used modern dosage is 15-40 g of Gualou(Trichosanthis Fructus),5 g of Huanglian(Coptidis Rhizoma),and 12 g of Banxia(Pinelliae Rhizoma).Xiaoxianxiong Decoction was originally used for treating tuberculosis and thorax disease,and it is a formula for clearing away heat and removing phlegm,relaxing the thorax,dispersing the tuberculosis,and its modern application is not restricted to the disease area of the heart.Modern clinical application is not confined to the location of the heart,but extended and expanded on the basis of ancient records,widely used in the treatment of pleurisy,acute bronchitis,emphysema,angina pectoris,acute and chronic gastritis,esophagitis,cholecystitis and other multi-systemic diseases.The main points to note when using this formula are as follows:(1) dosage,the dosage of Huanglian(Coptidis Rhizoma) should not be too large,to prevent bitter and cold diarrhea products sad Yang;(2) Banxia(Pinelliae Rhizoma) concoction,it is recommended to use alum for detoxification and preservation;(3) The fruit of Gualou(Trichosanthis Fructus) is used into the drug and first decocted,the whole fruit into the drug refers to the Gualou(Trichosanthis Fructus) with skin and kernels,which can enhance the role of the knot;(4) The key points of diagnosis and treatment are red tongue with yellow and greasy coating,smooth pulse,and.The formula can
