摘要
肝豆状核变性(Wlison’s disease,WD),是一种由ATP7B基因突变而导致的常染色体隐形遗传性疾病。其主要发病机制为铜蓝蛋白合成障碍,铜无法从胆汁排泄,沉积于各个脏器导致的病理变化。失眠是WD常见的一种非运动症状。中医认为失眠属于“不寐”范畴,多与心、肝、肾相关,心阳不振、肾水不足、肝郁化火均是不寐的病因,“肝心和合”理论最早被提出运用于“双心疾病”,近年来多被用来作为不寐的辨治思路。文章以中医理论和现代临床研究为基础,探讨了从“肝心和合”理论诊治WD失眠的依据和临床应用要点,为WD失眠的中医诊疗提供新的思路与方法。
Wilson’s disease(WD)is an autosomal recessive genetic disease caused by ATP7B gene mutation.Its main pathogenesis is the disorder of ceruloplasmin synthesis,copper cannot be excreted from bile,and copper is deposited in various organs,resulting in pathological changes.Insomnia is a common non-motor symptom of WD.Traditional Chinese medicine believes that insomnia belongs to the category of“insomnia”,which is often related to the heart,liver,and kidney.Heart yang deficiency,kidney water deficiency,and liver depression are all causes of insomnia.The“liver-heart harmony”theory was first proposed and applied to“double heart disease”.In recent years,it has been used as a treatment idea for insomnia.Based on traditional Chinese medicine theory and modern clinical research,this article discusses the basis and clinical application points of the“liver-heart harmony”theory for the diagnosis and treatment of WD insomnia,and provides new ideas and methods for the diagnosis and treatment of WD insomnia in traditional Chinese medicine.
