摘要
目的:研究中药热敷结合快速康复干预在胃肠道手术患者中的应用效果及对患者胃肠功能的影响。方法:选取行胃肠手术治疗的100例患者,分为对照组和研究组,每组50例。两组均行快速康复护理,在此基础上研究组使用中药热敷(海盐、吴茱萸)脾经、胃经,共干预7天。比较两组患者术后胃肠功能恢复状况、生活和睡眠质量,并统计并发症及满意度。结果:研究组胃肠功能较对照组恢复更显著,表现为肠鸣音恢复时间、首次排便时间、腹胀消失时间缩短(P<0.05);干预第3、7天时研究组胃肠功能评分较对照组降低(P<0.05),胃动素、胃泌素水平较对照组升高(P<0.05);干预第7天时研究组生活质量评分、日常活动评分、社交评分较对照组升高,睡眠质量评分较对照组降低(P<0.05);研究组腹胀、腹痛、便秘等发生率低于对照组,总满意度高于对照组(P<0.05)。结论:中药热敷结合快速康复干预可促进胃肠道手术患者术后胃肠蠕动,增加胃肠激素分泌,降低腹胀、腹痛发生率,进而缩短康复进程,提高生活质量、睡眠质量。
Objective:To study the application effects of hot compress with Chinese medicine joined with rapid rehabilitation intervention on patients undergoing gastrointestinal surgery and its impacts on gastrointestinal function.Methods:All 100 patients who accepted gastrointestinal surgery were chosen and divided into the control group and the study group with 50 cases in each group.Both groups received rapid rehabilitation nursing,based on this,the study group accepted hot compress with baysalt and Wuzhuyu(euodiae fructus)on spleen and stomach meridians,they were intervened for seven days.To compare the conditions of gastrointestinal function,the quality of life and sleep after the surgery between the two groups,and count the frequency and satisfaction degrees.Results:The improvement of the study group was more noticeable than that of the control group,manifesting the recovery time of borborygmus,the defeca-tion time and the time of abdominal distension shortening(P<0.05).After three and seven days of intervention,the scores of gastrointestinal function in the study group were lower than in the control group(P<0.05),the levels of motilin(MTL)and gastrin(GAS)were higher than in the control group(P<0.05),After seven days of the intervention,the scores of quality of life,daily activity scores and social rating in the study group were elevated than in the control group,the scores of sleep quality were reduced than in the control group(P<0.05).The study group was lower than the control group in the incidences of abdominal distension,abdominal pain and constipation,higher than the control group in overall satisfaction degrees(P<0.05).Conclusion:Hot compress with Chinese medicine joined with rapid rehabilitation intervention could shorten the rehabilitation process,improve quality of life and sleep quality through promoting gastrointestinal motility in patients after the operation,increasing the secretion of gastrointes-tinal hormone,and reducing the incidences of abdominal distension and abdominal pain.
