摘要
青光眼是一种由于眼压升高而导致的眼部疾病,视神经损伤是其主要病征之一,目前西医对于该病的治疗,仅以控制高眼压为主,对所造成的视神经损伤缺乏有效的方法,部分患者将眼压控制正常后,视神经损伤仍然继续进展,导致视功能持续恶化。因此,寻找在合理控制眼压后,延缓甚至逆转对视神经损伤进展的治疗方法,是重中之重。中医“从肝论治”青光眼视神经损伤具有一定特色,且其疗效显著。本文基于“肝开窍于目”理论,阐述肝主疏泄、肝主藏血与青光眼视神经损伤“机械压力学说”“缺血学说”两大发病机制之间的联系,并对从肝辨证论治该病的证候类型进行总结,拟为各位同行提供参考。
Glaucoma is an ocular disease caused by elevated intraocular pressure,with optic nerve damage being one of its main symptoms.Currently,Western Medicine primarily focuses on controlling high intraocular pressure for treatment,but there is a lack of effective methods for addressing optic nerve damage.In some patients,optic nerve damage continues to progress despite intraocular pressure control,leading to a sustained decline in visual function.Therefore,it is critical to explore treatment approaches that can delay or even reverse the progression of optic nerve damage after reasonable control of intraocular pressure.Traditional Chinese Medicine(TCM)offers a distinctive approach by treating glaucoma-induced optic nerve damage based on the concept of"Liver Opens to Eyes",and its effectiveness has been significant.This article elaborates on the connection between liver's functions of promoting the smooth flow of Qi,storing blood,and the two primary pathogenic mechanisms of glaucoma-induced optic nerve damage—the"mechanical pressure theory"and the"vascular ischemia theory".Additionally,it summarizes the syndrome types for the treatment of this disease based on liver differentiation,providing a reference for colleagues in the field.
