摘要
小儿厌食症是儿童常见的以食欲下降为主要表现的消化系统病症。本文介绍了黄甡教授在临床上基于肝脾同调论治小儿厌食症的诊疗思路和用药经验。黄甡教授认为小儿厌食症病机乃脾胃气虚、乳食积滞、脾虚肝旺、脾胃不和、脾胃湿热等所致,临床治疗以运脾疏肝为主线,佐以消食积、补脾气、滋胃阴、清里热之法,结合小儿的病理生理特点,以脾、肝同调为基础,总结出治疗小儿厌食症的基本处方,加减应用,使小儿脾实肝疏、脏腑通调。后附黄甡教授应用基本处方治疗小儿厌食症医案说明,为临床医师治疗小儿厌食症提供参考。
Infantile anorexia is a common digestive system disease characterized by loss of appetite in children.This article introduces the clinical syndrome diff erentiation and treatment ideas,prescription experience of Huang Shen in his treatment of infantile anorexia based on the liver and spleen harmonization theory.Professor Huang Shen believes that the pathogeneses of the disease involve spleen-stomach qi deficiency,milk and food retention and stagnation,spleen deficiency and liver qi hyperactivity,disharmony between the spleen and stomach,dampness-heat of the spleen and stomach.His clinical treatment underscores activating the spleen and dispersing the stagnated liver qi,supplemented by methods of resolving food retention and stagnation,tonifying spleen qi,nourishing stomach yin and clearing internal heat.Taking into account the pathophysiological characteristics of infants,this paper summarizes the basic therapeutic prescription for infantile anorexia based on the liver and spleen harmonization theory.The modified prescription can be applied in the treatment of infantile anorexia to strengthen the infants'spleen,disperse the stagnated liver qi,and to regulate and harmonize the organs as a whole.A case study of infantile anorexia patients treated with the basic therapeutic prescription by Professor Huang Shen is attached,which is expected to provide reference and trigger discussion among clinicians in this respect.
