摘要
目的:探讨血浆微小RNA-9-5p(microRNA-9-5p,miR-9-5p)表达水平预测奥氮平治疗精神分裂症疗效的价值。方法:纳入杭州市第七人民医院2021年6月至2023年6月收治的78例奥氮平单药治疗的精神分裂症患者为研究对象,连续观察4周,采用阳性与阴性症状量表(positive and negative syndrome scale,PANSS)评定疗效。采用实时荧光定量聚合酶链式反应法检测血浆miR-9-5p表达水平。结果:基线血浆miR-9-5p表达水平与PANSS量表阴性症状减分率、一般精神病理症状减分率及总减分率呈负相关(r=-0.248、-0.320、-0.360,P均<0.05),治疗后血浆miR-9-5p表达水平增加量与PANSS量表阳性症状减分率、阴性症状减分率、一般精神病理症状减分率及总减分率呈正相关(r=0.236、0.317、0.436、0.462,P均<0.05)。miR-9-5p基线表达水平预测奥氮平治疗精神分裂症疗效的曲线下面积(area under the curve,AUC)为0.7792(95%CI:0.6692~0.8892),灵敏度为63.33%,特异度为83.33%,miR-9-5p表达水平增加量预测奥氮平治疗精神分裂症疗效的AUC为0.8278(95%CI:0.7248~0.9308),灵敏度为79.17%,特异度为83.33%。生物信息学分析表明,miR-9-5p预测靶基因参与多种与神经系统功能密切相关的生物学过程,富集于ErbB信号通路、神经营养素信号通路、MAPK信号通路等通路。结论:血浆miR-9-5p表达水平一定程度上可以预测奥氮平治疗精神分裂症的临床疗效。
Objective:To investigate the value of plasma microRNA-9-5p(miR-9-5p)expression level in predicting the efficacy of olanzapine in the treatment of schizophrenia.Method:A total of 78 schizophrenic patients admitted to Hangzhou Seventh People's Hospital from June 2021 to June 2023 who treated with olanzapine alone for 4 weeks were recruited.Olanzapine treatment efficacy was measured by positive and negative symptom scale(PANSS).The relative expression of miR-9-5p in plasma was detected by real-time fluorescence quantitative polymerase chain reaction.Results:The baseline plasma miR-9-5p expression level was negatively correlated with the negative symptom reduction rate,general psychopathology reduction rate,and total reduction rate of PANSS(r=-0.248,-0.320,-0.360,all P<0.05).The upregulation of plasma miR-9-5p expression level after olanzapine treatment was positively correlated with the positive symptom reduction rate,negative symptom reduction rate,general psychopathology reduction rate,and total reduction rate of PANSS(r=0.236,0.317,0.436,0.462,all P<0.05).The baseline of plasma miR-9-5p expression level predicted the efficacy of olanzapine in the treatment of schizophrenia with the AUC of 0.7792(95%CI:0.6692~0.8892),sensitivity of 63.33%,specificity of 83.33%.The upregulation of plasma miR-9-5p expression level predicted the efficacy of olanzapine in the treatment of schizophrenia with the AUC of 0.8278(95%CI:0.7248~0.9308),sensitivity of 79.17%,specificity of 83.33%.Bioinformatics analysis showed that the predicted target genes of miR-9-5p involved in various biological processes closely related to nervous system function and enriched in pathways such as ErbB signaling pathway,neurotrophin signaling pathway and MAPK signaling pathway.Conclusion:The expression level of plasma miR-9-5p can predict the clinical efficacy of olanzapine in the treatment of schizophrenia to some extent.
