摘要
目的探讨慢性胃炎患者血清CX3C趋化因子配体1(CX3CL1)、环氧酶-1(COX-1)水平与胃黏膜损伤程度的关系。方法回顾性选择延安大学咸阳医院2021年9月至2023年9月收治的150例慢性胃炎患者为研究组,另选同时期进行健康体检的100例健康人员为对照组。将其按照肠化生(OLGIM)分期分为Ⅰ~Ⅳ期,并按萎缩(OLGA)分期分为Ⅰ~Ⅲ期;比较两组CX3CL1、COX-1、胃蛋白酶原Ⅰ(PGⅠ)、胃蛋白酶原Ⅱ(PGⅡ)、PGⅠ/PGⅡ水平,分析CX3CL1、COX-1水平与患者胃黏膜损伤程度之间的相关性,分析CX3CL1、COX-1水平联合检测对慢性胃炎患者胃黏膜损伤程度的预测价值。结果研究组CX3CL1、COX-1水平高于对照组[(155.34±12.11)ng/L比(95.47±7.35)ng/L、(19.51±1.50)μg/L比(12.35±1.22)μg/L](P<0.01);OLGA:与I期患者比较,Ⅱ期、Ⅲ期患者的COX-1、CX3CL1水平升高[(15.36±1.29)μg/L和(18.66±1.58)μg/L比(12.10±1.16)μg/L、(124.19±9.27)ng/L和(153.21±12.19)ng/L比(93.28±7.20)ng/L](P<0.01);与Ⅱ期患者比较,Ⅲ期患者的COX-1、CX3CL1水平较高,差异有统计学意义(P<0.01)。OLGIM:与Ⅰ期患者比较,Ⅱ、Ⅲ及Ⅳ期的CX3CL1和COX-1水平升高[(140.52±11.15)ng/L、(149.37±12.24)ng/L、(158.29±13.31)ng/L比(92.82±8.26)ng/L、(14.50±1.32)μg/L、(18.22±1.41)μg/L和(20.27±1.60)μg/L比(12.16±1.20)μg/L],差异有统计学意义(P<0.01);与Ⅱ期患者比较,Ⅲ期及Ⅳ期的COX-1、CX3CL1水平上升(P<0.01);与Ⅲ期患者比较,Ⅳ期的CX3CL1、COX-1水平升高(P<0.01);与对照组比较,研究组PGⅠ、PGⅠ/PGⅡ水平降低[(39.35±15.68)μg/L比(119.12±17.36)μg/L、2.77±1.29比16.87±1.09],且PGⅡ水平升高[(17.11±15.95)μg/L比(11.96±7.05)μg/L]。Pearson相关性分析显示,CX3CL1、COX-1水平与患者胃黏膜损伤程度呈正相关(P<0.01)。CX3CL1、COX-1两项联合预测慢性胃炎患者胃黏膜损伤程度的临床价值相比于单项预测明显升高(P<0.01)。结论观察血清CX3CL1、COX-1在慢性胃炎患者体内的水平
ObjectiveTo investigate the relationship between serum levels of CX3C chemokine ligand 1(CX3CL1),cyclooxygenase-1(COX-1)and the degree of gastric mucosal damage in patients with chronic gastritis.MethodsOne hundred and fifty patients with chronic gastritis admitted to Yan′an University,Xianyang Hospital from September 2021 to September 2023 were in the study group,and another 100 healthy patients who underwent physical examination during the same period were selected as the control group.And 100 patients with chronic gastritis were divided into stagesⅠ,Ⅱ,Ⅲ,andⅣaccording to intestinal metaplasia(OLGIM)staging,and into mild,moderate,and severe according to atrophy(OLGA)grading;the levels of CX3CL1 and COX-1 in each group were compared,the correlation between CX3CL1,COX-1 levels and the degree of gastric mucosal injury in patients were analyzed,the predictive value of combined detection of CX3CL1 and COX-1 levels for the degree of gastric mucosal injury in patients with chronic gastritis were analyzed.ResultsThe levels of CX3CL1 and COX-1 in the study group were higher than those in the control group:(155.34±12.11)ng/L vs.(95.47±7.35)ng/L,(19.51±1.50)μg/L vs.(12.35±1.22)μg/L(P<0.01);OLGA:compared with stageⅠpatients,the levels of COX-1 and CX3CL1 in stageⅡandⅢpatients increased:(15.36±1.29)μg/L and(18.66±1.58)μg/L vs.(12.10±1.16)μg/L,(124.19±9.27)ng/L and(153.21±12.19)ng/L vs.(93.28±7.20)ng/L(P<0.01);Compared with stageⅡpatients,stageⅢpatients have higher levels of COX-1 and CX3CL1(P<0.01).OLGIM:Compared with stageⅠpatients,the levels of CX3CL1 and COX-1 in stagesⅡ,Ⅲ,andⅣwere elevated:(140.52±11.15)ng/L,(149.37±12.24)ng/L and(158.29±13.31)ng/L vs.(92.82±8.26)ng/L;(14.50±1.32)μg/L,(18.22±1.41)μg/L and(20.27±1.60)μg/L vs.(12.16±1.20)μg/L(P<0.01);Compared with stageⅡpatients,the levels of COX-1 and CX3CL1 in stageⅢandⅣshowed an increase(P<0.01);Compared with stageⅢpatients,the levels of CX3CL1 and COX-1 in stageⅣincreased(P<0.01);Compared with the control
