摘要
目的:分析伴眩晕的单侧全聋型突发性聋(TD-SSNHL)患者的临床特征及预后情况,并探讨伴眩晕的单侧TD-SSNHL患者预后的影响因素。方法:选取82例伴眩晕的单侧TD-SSNHL患者作为研究对象,收集患者性别、年龄、患耳侧别、既往史(高血压、糖尿病等)、病程(发病至开始治疗时间)、伴随症状、视频头脉冲试验(vHIT)结果、冷热试验(CT)结果及治疗效果等,采用单因素及多因素Logistic回归模型分析单侧TD-SSNHL患者预后影响因素。结果:82例单侧TD-SSNHL患者经规范治疗后,痊愈率1.22%(1/82),总有效率54.88%(45/82)。治疗后眩晕、耳鸣、耳闷症状改善的总体有效率分别为100.00%、63.16%和93.75%。单因素分析显示:年龄、病程、vHIT结果、CT结果与患者预后结局有关(P<0.05)。多因素Logistic回归分析显示:病程长(OR=0.372)、vHIT结果异常(OR=0.349)、CT结果异常(OR=0.295)是单侧TD-SSNHL患者预后结局不良(治疗无效)的独立影响因素(P<0.05)。结论:从发病至开始治疗所经历的时间越长、vHIT结果异常、CT结果异常是伴眩晕的单侧TD-SSNHL预后不良的危险因素。
Objective:To analyze the clinical characteristics and prognosis of unilateral total-deafness sudden sensorineural hearing loss(TD-SSNHL)with vertigo,and to explore the influencing factors of prognosis of unilateral td-ssnhl patients with vertigo.Methods:A total of 82 patients with unilateral td-ssnhl with vertigo were selected as the research subjects.Gender,age,ear side,past history(hypertension,diabetes,etc.),onset to treatment time,accompanying symptoms,video head impulse test(vHIT)results,and caloric test test(CT)and treatment effect were collected.Univariate and multivariate Logistic regression models were used to analyze the prognostic factors of patients with unilateral td-ssnhl.Results:After standardized treatment,the cure rate of 82 patients with unilateraltd-ssnhl was 1.22%(1/82),and the total effective rate was 54.88%(45/82).After treatment,the overall effective rates of vertigo,tinnitus and tinnitus symptoms were 100.00%,63.16%and 93.75%respectively.Univariate analysis showed that age,course of disease,results of vhit and CT were related to the prognosis of patients(P<0.05).Multivariate Logistic regression analysis showed that the longer the course of disease(OR=0.372),the abnormal results of vhit(OR=0.349),and the abnormal results of CT(OR=0.295)were the independent influencing factors of poor prognosis(ineffective treatment)in patients with unilateral td-ssnhl(P<0.05).Conclusion:The longer the time from onset to treatment,the abnormal results of vhit and CT are the risk factors for poor prognosis of unilateral td-ssnhl with vertigo.
