摘要
目的:探讨中性粒细胞CD64指数(nCD64 index)对儿童肺炎支原体肺炎(MPP)合并细菌感染的预测价值。方法:选取82例MPP患儿为研究对象,按照是否合并细菌感染分为单纯肺炎支原体(MP)感染组(n=51)及MP合并细菌感染组(n=31);另选31名儿保正常者为正常对照组。比较各组对象nCD64 index及单纯MP感染组和MP合并细菌感染组患儿实验室指标;采用Spearman相关性分析nCD64 index与实验室指标的相关性;采用受试者工作特征(ROC)曲线分析nCD64 index对MP合并细菌感染的预测价值。结果:MP合并细菌感染组患儿nCD64 index、白细胞计数(WBC)、中性粒细胞绝对值(N)、C反应蛋白(CRP)、降钙素原(PCT)和乳酸脱氢酶(LDH)水平均高于单纯MP感染组(P<0.05)。相关性分析显示,nCD64 index与WBC、N和CRP水平正相关(P<0.05)。ROC曲线分析显示,nCD64 index对MPP合并细菌感染有较高预测价值(AUC=0.743,P<0.05),且与CRP联合预测的价值更高(AUC=0.787,P<0.05)。结论:MPP合并细菌感染儿童nCD64 index升高,对MPP合并细菌感染有较高预测价值,其与CRP联合预测的价值更高。
Objective:To investigate the early predictive value of the neutrophil CD64 index(nCD64 index)in children with Mycoplasma pneumoniae pneumonia(MPP)combined with bacterial infection.Methods:82 children with MPP were selected as the research subjects and divided into a simple Mycoplasma pneumoniae(MP)infection group(n=51)and an MP combined with bacterial infection group(n=31)according to whether they were infected with bacteria or not,and selected 31 individuals with normal child protection as the normal control group.The nCD64 index and laboratory indicators were compared between the two groups.The correlation between nCD64 index and laboratory indicators were investigated by Spearman correlation analysis,and the predictive value of nCD64 index for MP combined with bacterial infection was analyzed using receiver operating characteristic(ROC)curve.Results:The nCD64 index,white blood cell count(WBC),neutrophil count(N),C-reactive protein(CRP),procalcitonin(PCT),and lactate dehydrogenase(LDH)were high in the MP combined with bacterial infection group(P<0.05).Correlation analysis showed that there was a positive correlation between the nCD64 index and WBC,N,and CRP levels.ROC curve analysis showed that nCD64 index had high area under the ROC curve(AUC=0.743,P<0.05),the joint prediction with CRP had the largest predictive value,the AUC was 0.787(P<0.05).Conclusion:The early predictive value of the nCD64 index in children with MPP combined with bacterial infection is better than that of traditional infection indices.Furthermore,its predictive value increased when combined with CRP levels.
