摘要
胃癌前病变(PLGC)的及时发现与诊治在逆转和防止癌症发生中有重要意义。沈舒文教授结合临床经验,自创“三维六纲”理论运用于胃癌前病变诊疗过程,按照围线主诉辨病因,寒热感受辨病性,滞逆症态辨病势,参合舌脉辨虚实4个步骤辨清病因病机。本病病机为脾胃气阴亏虚,毒瘀交阻,治疗大法为虚实关联,补虚为本;滞损交加,转滞为通;解毒散瘀,清热为要。并举验案1则。
Timely detection and diagnosis and treatment of gastric precancerous lesions are of great significance in reversing and preventing the occurrence of cancer.Based on clinical experience,Professor SHEN Shuwen creates the“three-dimensional and six guiding principles for diagnosis”theory and applies it to the diagnosis and treatment of gastric precancerous lesions.He identifies the etiology and pathogenesis based on the four steps of differetiating the causes of the disease according to the main complaint of the peripheral line,distinguishing the nature of disease according to the perception of cold and heat,identifying the disease situation according to the the state of stagnation and reverse syndrome,and distinguishing the deficiency and excess by reffering to the tongue and pulse.The pathogenesis of this disease is the deficiency of spleen and stomach qi,the intersection of poison and stasis,and proposes the treatment principle of tonifying in the case of mixed deficiency and excess,reversing the staganation in the case of mixed stagantion and damage,and detoxifying and dispersing stasis,clearing the heat.1 proven case is attached.
