不同康复措施对脑卒中后足下垂疗效的网状Meta分析

摘要 目的:系统评价功能电刺激、肌电生物反馈、踝足矫形器、肌内效贴、重复经颅磁刺激5种不同康复疗法对脑卒中足下垂患者下肢运动功能的干预效果。方法:计算机检索EMbase、Web of Science、The Cochrane Library、CBM、PubMed、CNKI、WanFang Data和VIP数据库。搜索5种康复疗法治疗脑卒中后足下垂的随机对照试验,检索时限为从建库至2022年7月。由2名研究者独立筛选文献、提取资料并评价纳入研究的偏倚风险后,采用STATA软件及ADDIS软件进行网状Meta分析。结果:共纳入40个随机对照试验,包括2749例患者。Meta分析结果显示,在运动、平衡及日常生活能力方面,功能电刺激、踝足矫形器、肌电生物反馈的疗效均优于常规康复,踝足矫形器最优。结论:踝足矫形器在改善脑卒中足下垂患者下肢运动功能、平衡及日常生活能力方面较其他4种疗法更有优势,但受纳入文献数量及质量的限制,结论待更多高质量研究进一步验证。 Objective:To systematically evaluate the effect of five different rehabilitation therapies-functional electrical stimulation,electromyographic biofeedback,ankle-foot orthosis,Kinesiotex and repetitive transcranial magnetic stimulation-on lower limb motor function in patients with post-stroke foot drop.Methods:A computerized search was conducted in EMbase,Web of Science,The Cochrane Library,CBM,PubMed,CNKI,WanFang Data and VIP databases for randomized controlled trials treating post-stroke foot drop with the five rehabilitation therapies,from inception to July 2022.Two researchers independently screened literature,extracted data,and assessed the risk of bias in the included studies.Network Meta-analysis was performed using STATA and ADDIS software.Results:A total of 40 randomized controlled trials involving 2749 patients were included.Meta-analysis results indicated that in terms of movement,balance and daily living ability,functional electrical stimulation,ankle-foot orthosis and electromyographic biofeedback were superior to conventional rehabilitation,and ankle-foot orthosis was the best.Conclusion:Ankle-foot orthosis have more advantages than the other four therapies in improving lower limb motor function,balance and daily living ability in stroke patients with foot drop.However,due to the limitations of the quantity and quality of the included literatures,the conclusions need to be further verified by more high-quality studies.
