摘要
目的:采用Meta分析的方法比较透刺与常规针刺治疗周围性面瘫的临床疗效。方法:选用中国知网、万方数据库、维普数据库、PubMed、Cochrane library、Embase、Web of science 7个中英文数据库进行相关文献检索。检索时间为建库至2023年8月30日。对检索到的文献运用Endnote X9进行初筛后阅读全文进行复筛,运用Excel提取纳入文献的关键数据,并用Cochrane质量评价表进行质量评价,最后采用RevMan 5.4软件进行数据分析。结果:共检索1567篇文献,经筛查共纳入18篇文献。分析结果显示,透刺法在总有效率、治愈率及Sunnybrook面神经功能评分、面部残疾指数量表躯体功能指数量表(Facial Disability Index Physical Function Index scale,FDIP)评分方面均优于常规针刺,差异有统计学意义。经过亚组分析,急性期透刺的总有效率优于常规针刺[RR=1.23,95%CI(1.11,1.36),P=0.006],治愈率优于常规针刺[RR=1.73,95%CI(1.99,3.03),P=0.05],治疗过程中间断行针的总有效率高于单纯留针,两组差异有统计学意义[RR=1.12,95%CI(1.08,1.16),P<0.00001)。结论:在总有效率、治愈率、Sunnybrook面神经功能评分、FDIP评分方面,透刺治疗皆优于常规针刺,且在急性期进行透刺治疗优于常规针刺,间断行针优于单纯留针。
Objective:To compare the clinical effect of penetrating acupuncture and conventional acupuncture on peripheral Ofacial paralysis by Meta-analysis.Methods:The relevant studies in seven Chinese or English databases(CNKI,Wanfang,VIP,PubMed,Cochrane library,Embase,and Web of science)were searched.The search time was from the establishment of the database to August 30,2023.The retrieved literature was preliminarily screened by Endnote X9 and then re-screened by reading the full text.Excel was used to extract the key data of the included literature after screening,and the quality was evaluated by Cochrane quality evaluation table.Finally,RevMan 5.4 software was used for data analysis.Results:A total of 1567 articles were retrieved,and 18 articles were included for data analysis.The results showed that the penetrating needling was superior to the conventional acupuncture in the total effective rate,cure rate and Sunnybrook nerve function score,and the differences were statistically significant.After subgroup analysis,the effective rate of penetrating needling in acute stage was better than that of conventional acupuncture[RR=1.23,95%CI(1.11,1.36),P=0.006],the cure rate was better than that of conventional acupuncture[RR=1.73,95%CI(1.99,3.03),P=0.05].The effective rate of needle disconnection during treatment was higher than that of needle retention alone,with a statistical difference[RR=1.12,95%CI(1.08,1.16),P<0.00001].Conclusion:In the total effective rate,cure rate,Toronto facial nerve function score,facial disability index physical function,penetrating needling treatment is superior to conventional acupuncture,penetrating needling treatment is superior to conventional acupuncture in the acute stage,and intermittent needling is superior to simple needle retention.
