摘要
目的:探讨从厥阴角度治疗中晚期结直肠癌的临床意义,为中晚期结直肠癌从厥阴论治提供中医理论依据,从而指导临床治疗。方法:结合中医文献研究及现代医学对中晚期结直肠癌的认识,紧扣中晚期结直肠癌的病因病机和厥阴的生理病理特性,探究临床治疗中晚期结肠癌的理法方药。中晚期结直肠癌日久迁延不愈,耗伤机体正气,加之情志不遂,肝气郁结,气血壅滞,痰毒凝结,导致邪气羁留于肠道。厥阴为三阴之末,厥阴为阖,主收纳、涵养,乃育阴通阳之位,若厥阴失阖,不能涵养阴气,阴气无以潜藏,阳无以生,阴阳不相顺接,则脏腑功能紊乱,故出现寒热错杂之症状。结果:中晚期结直肠癌的发生常因久思忧郁伤及肝脾,肝脾失其运化疏泄之功,气机不畅,瘀毒蕴结,浸淫肠道而成。中晚期结直肠癌的根本病位在厥阴,病机为厥阴阖机不调,故多表现为虚实错杂之证,治疗时应重视寒温并用、通调阴阳。结论:在中晚期结直肠癌的临床治疗上,主张用张机(字仲景)《伤寒论》厥阴病方乌梅丸,并附病案1则。乌梅丸辛开苦降以调畅全身气机,补虚泻实以调理脏腑虚实,从而使脏腑功能协调,阴阳平衡,运用乌梅丸治疗中晚期结直肠癌颇具临床指导意义。
Objective:To explore the clinical significance of treating middle and advanced colorectal cancer from the perspective of Jueyin(厥阴),so as to provide TCM theoretical basis for the treatment of middle and advanced colorectal cancer from the perspective of Jueyin,and to guide the clinical treatment.Methods:Based on the study of TCM literature and the understanding of modern medicine on the middle and advanced colorectal cancer,the etiology and pathogenesis of middle and advanced colorectal cancer and the physiological and pathological characteristics of Jueyin are closely discussed.If middle and advanced colorectal cancer is prolonged and does not heal,it will deplete the body’s healthy Qi(气),coupled with mood failure,liver Qi stagnation,Qi and blood obstruction,phlegm toxin coagulation,evil Qi will be detained in the intestinal tract and lead to cancer.Jueyin is the end of the channels and collaterals.Jueyin is the closure,and the main function is to absorb and conserve,which is to nourish Yin(阴)and release Yang(阳).If Jueyin is not shut,it can not nourish Yin and Qi,and Yin and Qi can not be hidden,Yang can not be born,and if Yin and Yang fail to connect,the function of viscera will be disordered,there would be the symptoms of mixed cold and heat.Results:The occurrence of middle and advanced colorectal cancer is often caused by injury to the liver and spleen due to prolonged depression resulting in the loss of the function of transportation and dispersal of liver and spleen,obstruction of Qi,accumulation of blood stasis and toxin,and immersion in the intestinal tract.The basic pathological position of middle and advanced colorectal cancer is Jueyin,and the pathogenesis is the disorder of Jueyin closure,so it is often manifested as the syndrome of mixed deficiency and accumulation.The treatment should pay attention to the combination of cold and warm,regulating Yin and Yang.Conclusion:In the clinical treatment of middle and advanced colorectal cancer,ZHANG Ji’s Jueyin disease prescription—Wumei Wan(�
