摘要
目的分析慢性阻塞性肺疾病(COPD)合并支气管哮喘(哮喘)患者血清25-羟基维生素D[25-(OH)D]与免疫功能、肺功能的关系。方法本研究为病例对照研究。采用非随机抽样法(配额抽样), 对康复大学青岛中心医院2021年1月至2023年10月收治的65例COPD合并哮喘患者进行研究, 根据25-(OH)D水平分为25-(OH)D正常组(n=25)和25-(OH)D降低组(n=40), 另纳入同期50名健康者为对照组。比较3组血清25-(OH)D水平、免疫功能指标和肺功能指标, 通过Spearman秩相关分析COPD合并哮喘患者血清25-(OH)D水平与免疫功能及肺功能的相关性。结果 25-(OH)D正常组男13例(52.0%), 女12例(48.0%), 年龄(69.98±8.78)岁;25-(OH)D降低组男21例(52.5%), 女19例(47.5%), 年龄(70.11±8.65)岁;对照组男30名(60.0%), 女20名(40.0%), 年龄(70.05±8.32)岁。3组性别、年龄差异无统计学意义(均P>0.05)。25-(OH)D降低组的血清25-(OH)D水平[(27.65±3.26)μg/L]低于25-(OH)D正常组[(41.98±5.26)μg/L]和对照组[(42.48±5.35)μg/L](F=132.80, P<0.001)。25-(OH)D降低组CD4^(+)[(27.65±3.26)%]、CD4^(+)/ CD8^(+)(0.84±0.07)低于25-(OH)D正常组[(35.65±4.35)%、1.12±0.11], 25-(OH)D正常组CD4^(+)、CD4^(+)/CD8^(+)低于对照组[(42.48±5.35)%、1.64±0.16];25-(OH)D降低组CD8^(+)[(32.98±4.35)%]高于25-(OH)D正常组[(29.65±4.12)%], 25-(OH)D正常组CD8^(+)高于对照组[(25.86±3.35)%](F值分别为120.54、475.88、37.47, 均P<0.001)。25-(OH)D降低组用力肺活量(FVC)[(2.56±0.36)L]、第1秒用力呼气容积(FEV_(1))[(1.58±0.16)L]、FEV_(1)/FVC[(61.72±4.44)%]低于25-(OH)D正常组[(2.92±0.41)L、(1.97±0.23)L、(67.47±1.78)%], 25-(OH)D正常组FVC、FEV_(1)、FEV_(1)/FVC低于对照组[(3.78±0.32)L、(2.98±0.54)L、(78.84±1.69)%](F值分别为139.15、157.27、386.10, 均P<0.001)。COPD合并哮喘患者血清25-(OH)D水平与肺功能指标及CD4^(+)、CD4^(+)/CD8^(+)呈正相关, 与CD8^(+)呈负相关(均P<0.05)。结论 COPD合并哮喘患者血清25-(OH)D水平与免疫�
Objective:To analyze the correlation of serum 25 hydroxyvitamin D[25-(OH)D]with immune function and pulmonary function in patients with chronic obstructive pulmonary disease(COPD)complicated with bronchial asthma.Methods:This study was a case-control study.From January 2021 to October 2023,65 COPD patients with bronchial asthma admitted to Qingdao Central Hospital,University of Health and Rehabilitation Sciences were included by non-random sampling(quota sampling)and divided into a reduced group(n=40)and a normal group(n=25)based on 25-(OH)D levels.Additionally,50 healthy individuals during the same period were included as the control group.Serum 25-(OH)D levels,immune function and pulmonary function indicators were compared among the three groups.The correlation of serum 25-(OH)D levels with immune function and pulmonary function in COPD patients with bronchial asthma was detected through Spearman rank correlation analysis.Results:There were 13(52.0%)males and 12(48.0%)females with a mean age of(69.98±8.78)years in the normal group.There were 21(52.5%)males and 19(47.5%)females with a mean age of(70.11±8.65)years in the reduced group.In the control group,there were 30(60.0%)males and 20(40.0%)females with a mean age of(70.05±8.32)years.No significant differences in gender and age were found among the three groups(all P>0.05).Serum 25-(OH)D levels in the reduced group([27.65±3.26]μg/L)were significantly lower than those in the normal group([41.98±5.26]μg/L)and the control group([42.48±5.35]μg/L)(F=132.80,P<0.001).The CD4^(+)and CD4^(+)/CD8^(+)in the reduced group([27.65±3.26]%and 0.84±0.07,respectively)were lower than those in the normal group([35.65±4.35]%and 1.12±0.11,respectively).The CD4^(+)and CD4^(+)/CD8^(+)in the normal group were significantly lower than those in the control group([42.48±5.35]%and 1.64±0.16,respectively).The CD8^(+)in the reduced group([32.98±4.35]%)was significantly higher than that in the normal group([29.65±4.12]%),which in the normal group was higher than that in the
