摘要
目的探究补气、活血、通络法治疗糖尿病性周围神经病变(DPN)患者具体疗效。方法选取河北省第八人民医院2021年1月-2023年5月DPN患者(气虚型、血瘀型)共计100例,随机分为对照组与研究组各50例。两组患者入院均接受西医基础治疗,研究组在西医治疗的基础上加用补气、活血、通络方剂。比较两组患者治疗后2周、1个月、3个月的临床疗效以及神经传导速度的变化。结果(1)治疗过程中,两组患者空腹血糖(FPG)和餐后2小时血糖(2 hPG)、糖化血红蛋白(HbA1c)水平逐渐下降(P<0.05),对照组患者同时段FPG、2 hPG、HbAlc与研究组患者相比差异无统计学意义(P>0.05)。(2)治疗过程中,对照组患者右侧肢体腓总神经的运动神经传导速度(MCV)、正中神经MCV、右侧肢体腓总神经的感觉神经传导速度(SCV)、正中神经SCV未发生明显变化(P>0.05),研究组患者右侧肢体腓总神经MCV、正中神经MCV、右侧肢体腓总神经SCV、正中神经SCV逐渐增加,且明显高于同时段的对照组患者(P<0.05)。(3)治疗前,两组患者中医症候积分、MDNS差异无统计学意义(P>0.05);治疗后,研究组中医症候积分、MDNS明显低于对照组(P<0.05)。(4)研究组总有效46例(92.00%)明显高于对照组36例(72.00%)(P<0.05)。结论补气活血通络方可有效改善DPN患者神经传导速度,缓解患者症状,同时不影响西医降糖的治疗。
Objective To explore the specific efficacy of qi tonifying,blood activating and collaterals unblocking methods in the treatment of diabetes peripheral neuropathy(DPN).Methods A total of 100 DPN patients(Qi deficiency type and blood stasis type)from January 2021 to May 2023 in the eighth people′s hospital of Hebei were selected and randomly divided into a control group and a study group with 50 cases each.Both groups of patients received basic western medicine treatment upon admission,while the research group received additional prescriptions for tonifying qi,promoting blood circulation,and unblocking collaterals on the basis of western medicine treatment.Compare the clinical efficacy and changes in nerve conduction velocity between two groups of patients at 2 weeks,1 month,and 3 months after treatment.Results(1)During the treatment process,the levels of fasting blood glucose(FPG),blood glucose 2 hours after meal(2 hPG),and glycated hemoglobin(HbAlc)in the two groups of patients gradually decreased(P<0.05),while there was no significant difference in FPG,2 hPG,and HbAlc between the control group and the study group at the same time(P>0.05).(2)During the treatment process,there was no significant change in the conduction velocity of the right common peroneal motor nerve condiction velocity(MCV),median nerve MCV,right common peroneal sensory nerve conolution velocity(SCV),and median nerve SCV in the control group patients(P>0.05).(3)The conduction velocity of the right common peroneal nerve MCV,median nerve MCV,right common peroneal nerve SCV,and median nerve SCV in the study group patients gradually increased,and was significantly higher than that of the control group patients in the same period(P<0.05).Before treatment,there was no significant difference in TCM symptom scores and MDNS between the two groups of patients(P>0.05);after treatment,the TCM symptom scores and MDNS in the study group were significantly lower than those in the control group(P<0.05).(4)The total effective rate in the study group(46 cases,
