摘要
目的探讨血清降钙素原(procalcitonin,PCT)、C反应蛋白(C-reactive protein,CRP)、淀粉样蛋白A(serum amyloid A,SAA)和白细胞介素6(interleukin 6,IL-6)水平在重症肺炎患儿中的变化情况及临床意义。方法非随机选取2020年2月—2023年2月期间张掖市第二人民医院、河西学院附属张掖人民医院、张掖市甘州区人民医院收治的134例肺炎患儿,并作为观察组;根据病情判断标准将其分为轻症组(n=68)与重症组(n=66)。同时纳入在同一时期接受体检的130名健康儿童作为对照组。分别对比不同组的血清PCT、CRP、SAA以及IL-6水平;采用Pearson对血清PCT、CRP、SAA以及IL-6水平和病情程度的相关性进行分析。结果观察组的血清PCT、CRP、SAA以及IL-6水平均高于对照组,轻症组患儿的血清PCT、CRP、SAA以及IL-6水平均低于重症组,差异有统计学意义(P均<0.05);Pearson分析后可见PCT、CRP、SAA、IL-6水平变化与重症肺炎患儿的病情变化具有相关性,且均为正相关(r=0.237、0.300、0.266、0.223,P均<0.05)。结论通过对患儿血清中的PCT、CRP、SAA、IL-6含量的联合测定,可为重度肺炎的早期诊断、早期干预、延缓疾病发展、改善预后等方面提供重要的理论依据。
Objective To investigate the changes and clinical significance of serum procalcitonin(PCT),C-reactive protein(CRP),serum amyloid A(SAA)and interleukin-6(IL-6)levels in children with severe pneumonia.Methods 134 cases of children with severe pneumonia admitted to Zhangye Second People's Hospital,Zhangye People's Hospital Affiliated to Hexi University,and Zhangye Ganzhou District People's Hospital during the period from February 2020 to February 2023 were non-randomly selected,and served as the observation group;they were divided into the mild group(n=68)and the severe group(n=66)according to the criteria of judging the condition.At the same time,130 healthy children who underwent medical examination during the same period were included as the control group.The serum PCT,CRP,SAA and IL-6 levels of the different groups were compared,and Pearson was used to analyze the correlation between the serum PCT,CRP,SAA and IL-6 levels and the degree of the disease.Results The serum PCT,CRP,SAA and IL-6 levels in the observation group were higher than those in the control group,the serum PCT,CRP,SAA and IL-6 levels of the children in the mild group were lower than those of the children in the severe group,the differences were statistically significant(all P<0.05).The changes in the levels of PCT,CRP,SAA and IL-6 were correlated with the changes in the condition of the children with severe pneumonia,and all of them were positively correlated(r=0.237,0.300,0.266,0.223,all P<0.05).Conclusion The combined measurement of PCT,CRP,SAA,and IL-6 levels in children's serum can provide an important theoretical basis for early diagnosis,early intervention,delaying disease progression and improving the prognosis of severe pneumonia.
