摘要
乳腺癌是女性常见的恶性肿瘤,系统性辅助化疗可明显提高患者生存率。但化疗相关认知障碍(CRCI)是该病常见并发症,主要表现为注意力不集中、情绪障碍、思维及判断力下降等,严重影响患者生活质量。随着磁共振成像(MRI)技术迅速发展,多模态MRI、影像组学是研究乳腺癌CRCI发病机制及预后评估的有效手段,并获得诸多成果。多模态MRI可显示灰质体积、皮层厚度、白质微结构、脑代谢、脑血流灌注、脑区激活和功能连接变化等;MRI影像组学可提取和挖掘图像中的高维数据。本文对上述内容进行综述,探索临床意义,旨在为乳腺癌CRCI神经机制研究提供新思路和新策略。
Breast cancer is a common malignancy among women,and systemic adjuvant chemotherapy can significantly improve patient survival rates.However,chemotherapy-related cognitive impairment(CRCI)is a frequent complication of this disease,manifested primarily as inattention,emotional disturbances,decline in thinking and judgment abilities,significantly impacting patients’quality of life.With the rapid development of magnetic resonance imaging(MRI)technology,multimodal MRI and radiomics have emerged as effective tools for studying the pathogenesis and prognostic assessment of breast cancer-related CRCI,yielding numerous achievements.Multimodal MRI can reveal changes in gray matter volume,cortical thickness,white matter microstructure,brain metabolism,cerebral blood flow perfusion,brain region activation,and functional connectivity;MRI radiomics can extract and mine high-dimensional data from images.This article reviews the above contents,explores clinical significance,and aims to provide new ideas and strategies for the study of the neural mechanism of CRCI in breast cancer.
