摘要
目的:分析糖尿病阴虚体质人群的血清代谢组学、红外线热像特征,并探讨其与血清炎症因子的相关性。方法:选取43例糖尿病阴虚体质患者作为糖尿病阴虚质组,另选择43例健康平和质人群作为健康平和质组。比较2组各功能区红外线热像温度ΔT值、血清肿瘤坏死因子-α(TNF-α)及白细胞介素-6(IL-6)水平,并运用Pearson检验分析糖尿病阴虚体质组各功能区红外线热像温度ΔT值与炎性因子水平的相关性;采用液相色谱-质谱(LC-MS)技术检测组间差异代谢物,运用主成分分析(PCA)、偏最小二乘判别分析(PLS-DA)等方法结合Metaboanalyst5.0软件对关键代谢通路进行聚焦。结果:研究过程中,健康平和质组剔除3例,最终完成研究40例。糖尿病阴虚体质组任脉、督脉、手心、头面、胸膺、胃脘、肾区温度ΔT值及血清TNF-α、IL-6水平高于健康平和质组(P<0.05),少腹温度ΔT值降低(P<0.05)。糖尿病阴虚体质组各功能区红外线热像温度ΔT值与TNF-α水平无相关性(P>0.05),任脉、手心、头面红外线热像温度ΔT值均与IL-6水平成正相关(P<0.05)。代谢组学研究发现糖尿病阴虚质人群血清中出现15种变化明显的差异代谢物,包括棕榈酸、L-哌啶酸、脱氧核糖、葡萄糖等,主要涉及花生四烯酸代谢、戊糖磷酸途径、D-谷氨酰胺和D-谷氨酸代谢、半乳糖代谢、脂肪酸生物合成代谢通路,其中花生四烯酸代谢通路的关联性最强。结论:糖尿病阴虚质人群有任督脉热性与“上热下寒”的红外线热像特点,且血清IL-6升高水平与任脉、手心、头面偏热相关。糖尿病阴虚质发病与花生四烯酸代谢通路、半乳糖代谢、脂肪酸生物合成、戊糖磷酸途径、D-谷氨酰胺和D-谷氨酸代谢5个代谢通路有关,其中花生四烯酸代谢通路中的AA-CYP-EETs代谢轴可能是最重要的作用途径。
Objective:To analyze the serum metabolomics and infrared thermography characteristics of diabetes mellitus patients of yin deficiency constitution,and to explore the correlation between them and serum inflammatory factors.Methods:A total of 43 diabetes mellitus patients of yin deficiency constitution were selected as“the group of diabetes mellitus patients of yin deficiency constitution”,and 43 healthy and neutral people were chosen as“the group of healthy and neutral constitution”.TheΔT values of infrared thermal image temperature and levels of serum tumor necrosis factor-α(TNF-α)and interleukin-6(IL-6)in each functional area in the two groups were compared,and the correlation betweenΔT values of infrared thermal image temperature in each functional area and inflammatory factors levels in the group of diabetes mellitus patients of yin deficiency constitution were analyzed by Pearson Test.The differential metabolites between the two groups were detected by liquid chromatogre-mass spectrometry(LC-MS),and the key metabolic pathways were focused by principal component analysis(PCA),partial least squares discriminant analysis(PLS-DA)and Metaboanalyst 5.0 software.Results:In the course of the study,three cases in the group of healthy and neutral constitution were excluded,and 40 cases completed the study eventually.In the group of diabetes mellitus patients of yin deficiency constitution,theΔT value of conception vessel,governor vessel,palm,head and face,thoracic cavity,stomach cavity and kidney areas,and the levels of serum TNF-αand IL-6 were higher than those in the group of healthy and neutral constitution(P<0.05),and theΔT value of hypoabdominal temperature was decreased(P<0.05).There was no correlation between theΔT value of infrared thermal image in each functional area and TNF-αlevels in the group of diabetes mellitus patients of yin deficiency constitution(P>0.05),but there was a positive correlation between theΔT value of infrared thermal image of conception vessel,palm,head and face and IL-
