摘要
目的探讨术前中药保留灌肠结合回流灌肠治疗先天性巨结肠(Hirschsprung’s disease,HD)患儿的临床价值。方法选取2016年5月至2021年12月赣南医学院第一附属医院收治的48例HD择期手术患儿作为研究对象,按照信封盲抽方式分为两组,每组24例。对照组给予回流灌肠处理,实验组在对照组基础上联合术前中药保留灌肠治疗,比较两组灌肠效果、恢复时间指标、灌肠指标、并发症发生情况。结果实验组灌肠优良率为87.50%,高于对照组的62.50%,差异有统计学意义(P<0.05)。实验组开始进食时间、正常进食时间、住院时间均短于对照组,差异有统计学意义(P<0.05)。实验组灌肠时间短于对照组,每次灌洗入量、每次灌洗出量均多于对照组,每次残留液量均少于对照组,差异有统计学意义(P<0.05)。两组灌肠并发症发生率比较差异无统计学意义。实验组近期并发症发生率低于对照组,差异有统计学意义(P<0.05);两组远期并发症发生率比较差异无统计学意义。结论术前中药保留灌肠结合回流灌肠治疗HD效果可,可有效提高灌肠效果,减少并发症,缩短康复周期,值得临床推广应用。
Objective To explore the clinical value of preoperative Traditional Chinese Medicine retention enema combined with reflux enema in children with Hirschsprung's disease(HD).Methods 48 children with elective HD surgery admitted to the First Affiliated Hospital of Gannan Medical College from May 2016 to December 2021 were selected as the study subjects,and they were divided into the two groups,with 24 cases in each group.The control group was given reflux enema,and the experimental group was treated with Traditional Chinese Medicine retention enema before operation on the basis of the control group,the enema effect,recovery time index,enema index and occurrence of complications were compared between the two groups.Results The excellent rate of enema in the experimental group was 87.50%,which was higher than 62.50%in the control group,and the difference was statistically significant(P<0.05).The start time of eating,normal eating time and hospitalization time in the experimental group were all shorter than those in the control group,and the differences were statistically significant(P<0.05).The enema time in the experimental group was shorter than that in the control group,and the amount of lavage in and out each time was higher than that in the control group,the amount of residual fluid in each lavage was lower than that in the control group,and the difference was statistically significant(P<0.05).There was no statistically significant difference in the incidence of enema complications between the two groups.The incidence of recent complications in the experimental group was lower than that in the control group,and the difference was statistically significant(P<0.05).There was no statistically significant difference in the incidence of long-term complications between the two groups.Conclusion The combination of preoperative Traditional Chinese Medicine retention enema and reflux enema is effective in treating HD,which can effectively improve the enema effect,reduce complications,and shorten the rehabilitation period,
