血清谷氨酰转移酶和碱性磷酸酶在急性胆囊炎中的变化及其临床意义

Changes of serum glutamyltransferase and alkaline phosphatase in acute cholecystitis and their clinical significance

摘要 目的:探讨血清谷氨酰转移酶和碱性磷酸酶在急性胆囊炎中的变化及其临床意义。方法:选取2024年1月至2024年7月黄淮学院附属驻马店市中心医院收治的159例急性胆囊炎患者作为研究对象,同时选取159例健康体检者作为对照研究。患者入院后抽取外周血,检测血清谷氨酰转移酶和碱性磷酸酶的水平。根据患者病情严重程度,将急性胆囊炎患者分为轻度组、中度组和重度组。采用受试者工作特征曲线分析血清谷氨酰转移酶和碱性磷酸酶水平诊断急性胆囊炎的价值,采用皮尔森相关系数分析血清谷氨酰转移酶和碱性磷酸酶水平与急性胆囊炎的相关水平。组间计量数据比较采用独立样本t检验。结果:对照组患者血清谷氨酰转移酶和碱性磷酸酶水平[(33.16±10.20)、(89.91±19.15)U/L]明显低于观察组[(95.00±32.73)、(164.78±21.15)U/L],差异有统计学意义(t=22.740、25.820,P<0.05)。轻度组患者血清谷氨酰转移酶和碱性磷酸酶水平[(59.65±8.76)、(134.71±12.90)U/L]明显低于中度组[(90.47±13.87)、(160.70±12.57)U/L],差异有统计学意义(t=13.590、16.380,P<0.05)。中度组患者血清谷氨酰转移酶和碱性磷酸酶水平[(90.47±13.87)、(160.70±12.57)U/L]明显低于重度组[(133.48±13.28)、(197.74±23.80)U/L],差异有统计学意义(t=10.460、10.040,P<0.05)。血清谷氨酰转移酶和碱性磷酸酶是诊断急性胆囊炎的最佳截断值,曲线下面积分别为0.892、0.884,敏感度分别为0.714、0.784,特异度分别为0.725、0.812。血清谷氨酰转移酶和碱性磷酸酶联合检测急性胆囊炎的敏感度和特异性分别为0.849、0.822。血清谷氨酰转移酶和碱性磷酸酶与急性胆囊炎严重程度呈正相关(r=0.411、0.598,P<0.05)。结论:血清谷氨酰转移酶和碱性磷酸酶在急性胆囊炎患者血清中显著增加,对重症急性胆囊炎具有很好的诊断价值,两者联合检测可显著提高急性胆囊炎的临床诊断有效� Objective:To investigate the changes and clinical significance of serum glutamyltransferase and alkaline phosphatase in acute cholecystitis.Methods:159 patients with acute cholecystitis admitted to Zhumadian Central Hospital Affiliated to Huanghuai University from January to July 2024 were selected as the study objects,and 159 healthy subjects were selected as the control study.The levels of serum glutamyltransferase and alkaline phosphatase were detected by peripheral blood samples after admission.According to the severity of the disease,acute cholecystitis patients were divided into mild group,moderate group and severe group.The value of serum glutamyltransferase and alkaline phosphatase levels in the diagnosis of acute cholecystitis was analyzed by subject work characteristic curve,and the correlation between serum glutamyltransferase and alkaline phosphatase levels and acute cholecystitis was analyzed by Pearson correlation coefficient.Measurement data were compared between groups using independent samples t-test.Results:The levels of serum glutamyltransferase and alkaline phosphatase in control group[(33.16±10.20),(89.91±19.15)U/L]were significantly lower than those in observation group[(95.00±32.73),(164.78±21.15)U/L,t=22.740,25.820,P<0.05].The levels of serum glutamyltransferase and alkaline phosphatase in mild group[(59.65±8.76),(134.71±12.90)U/L]were significantly lower than those in moderate group[(90.47±13.87),(160.70±12.57)U/L,t=13.590,16.380,P<0.05].Serum glutamyltransferase and alkaline phosphatase levels in the moderate group[(90.47±13.87),(160.70±12.57)U/L]were significantly lower than those in the Severe group[(133.48±13.28),(197.74±23.80)U/L,t=10.460,10.040,P<0.05].Serum glutamyltransferase and alkaline phosphatase were the best cutoff values for the diagnosis of acute cholecystitis.The area under the curve was 0.892 and 0.884,the sensitivity was 0.714 and 0.784,and the specificity was 0.725 and 0.812,respectively.Serum glutamyltransferase and alkaline phosphatase were positively cor
