摘要
目的探讨基于快速康复外科(fast track surgery,FTS)理念的护理干预结合全程营养指导对妊娠期糖尿病(gestational diabetes mellitus,GDM)患者剖宫产术后糖代谢水平的影响。方法回顾性选取2021年3月—2023年2月东莞市妇幼保健院行剖宫产手术的90例GDM患者的临床资料,根据不同护理方案分为两组,各45例。对照组实施常规围手术期护理干预,观察组在对照组基础上实施基于FTS的护理干预结合全程营养指导,比较两组糖代谢指标、术后康复指标、并发症发生率。结果术后,两组空腹血糖、餐后2 h血糖水平均降低,且观察组均低于对照组,差异有统计学意义(P均<0.05);观察组术后进食时间、下床活动时间、肛门排气时间均短于对照组,差异有统计学意义(P均<0.05);观察组术后并发症发生率低于对照组,差异有统计学意义(P<0.05)。结论基于FTS的护理干预结合全程营养指导可改善GDM剖宫产患者的术后糖代谢水平,促进术后恢复,且减少并发症发生。
Objective To explore the effect of nursing intervention based on the concept of fast track surgery(FTS)combined with whole-course nutrition guidance on glucose metabolism level in patients with gestational diabetes mellitus(GDM)after cesarean section.Methods The clinical data of 90 patients with GDM who underwent cesarean section in Dongguan Maternal and Child Health Hospital from March 2021 to February 2023 were retrospectively selected.According to different nursing schemes,they were divided into two groups,45 cases in each group.The control group was given routine perioperative nursing intervention,and the observation group was given FTS-based nursing intervention combined with whole-course nutritional guidance on the basis of the control group.The glucose metabolism indexes,postoperative rehabilitation indexes and incidence of complication were compared between the two groups.Results After operation,the levels of fasting plasma glucose and 2-hour postprandial plasma glucose in the two groups decreased,and those in the observation group were lower than the control group,the differences were statistically significant(both P<0.05).The postoperative feeding time,ambulation time and anal exhaust time in the observation group were shorter than those in the control group,and the differences were statistically significant(all P<0.05).The incidence of postoperative complications in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion Nursing intervention based on FTS combined with whole-course nutritional guidance can improve the levels of postoperative glucose metabolism in patients with GDM undergoing cesarean section,promote postoperative recovery,and reduce complications.
