摘要
目的探讨饮食运动干预、二甲双胍联合胰岛素对妊娠期糖尿病患者妊娠结局及血糖控制的影响。方法选取2023年2月—2024年2月聊城市东昌府区妇幼保健院收治的104例妊娠期糖尿病患者为研究对象,根据不同的治疗方法分为对照组和观察组,各52例。对照组采取饮食运动干预与二甲双胍治疗,观察组在对照组基础上联合门冬胰岛素治疗。比较两组血糖指标、胰岛素抵抗指标和分娩方式及不良妊娠结局。结果治疗后,观察组空腹血糖、餐后2 h血糖、糖化血红蛋白水平均低于对照组,差异有统计学意义(P均<0.05)。治疗后,与对照组比较,观察组胰岛素抵抗指数更低,胰岛β细胞功能指数更高,差异有统计学意义(P均<0.05)。观察组剖宫产率低于对照组,差异有统计学意义(P<0.05)。两组不良妊娠结局发生率比较,差异无统计学意义(P>0.05)。结论妊娠期糖尿病患者采取饮食运动干预、二甲双胍联合胰岛素治疗可恢复正常血糖水平,改善胰岛素抵抗现状,利于患者自然分娩。
Objective To investigate the effects of diet and exercise intervention,metformin combined with insulin on pregnancy outcome and blood glucose control in patients with gestational diabetes mellitus.Methods A total of 104 patients with gestational diabetes mellitus admitted to Liaocheng Dongchangfu District Maternal and Child Health Hospital from February 2023 to February 2024 were selected as the research objects.According to different treatment methods,they were divided into control group and observation group,with 52 cases in each group.The control group was treated with diet and exercise intervention and metformin,and the observation group was treated with insulin aspart on the basis of the control group.The blood glucose indexes,insulin resistance indexes,delivery mode and adverse pregnancy outcome were compared between the two groups.Results After treatment,the levels of fasting plasma glucose,2-hour postprandial plasma glucose and glycated hemoglobin A1c in the observation group were lower than those in the control group,and the differences were statistically significant(all P<0.05).After treatment,compared with the control group,the insulin resistance index of the observation group was lower,and the isletβcell function index was higher,and the differences were statistically significant(both P<0.05).The cesarean section rate of the observation group was lower than that of the control group, and the difference was statistically significant (P<0.05). There was no significant difference in the incidence of adverse pregnancy outcomes between the two groups (P>0.05). Conclusion Gestational diabetes pregnant women take diet and exercise intervention, metformin combined with insulin therapy can restore normal blood glucose levels, improve the status of insulin resistance, conducive to maternal natural childbirth.
