摘要
目的探究胰岛素泵在妊娠期糖尿病(gestational diabetes mellitus,GDM)患者控糖治疗中的应用安全性。方法选取2022年2月—2024年2月厦门弘爱妇产医院收治的106例GDM患者为研究对象,按治疗方法的不同分为参照组(胰岛素多次注射)、观察组(胰岛素泵输注),各53例。对比两组糖代谢[空腹血糖(fasting blood glucose,FBG)、餐后2 h血糖(2-hour postprandial blood glucose,2 hPBG)、糖化血红蛋白(glycated hemoglobin A1c,HbA1c)]水平、胰岛功能[胰岛素抵抗指数(homeostasis model assessment-index resistance,HOMA-IR)、胰岛β细胞功能指数(homeostasis model assessment-β,HOMA-β)]及不良反应发生情况。结果分娩前,观察组HbA1c、FBG、2 hPBG水平均低于参照组,差异有统计学意义(P均<0.05)。分娩前,观察组HOMA-β高于参照组,HOMA-IR低于参照组,差异有统计学意义(P均<0.05)。观察组不良反应发生率为1.89%(1/53),低于参照组的15.09%(8/53),差异有统计学意义(χ^(2)=5.950,P<0.05)。结论胰岛素泵应用于GDM患者治疗可提高控糖效果,改善胰岛功能,降低不良反应发生风险。
Objective To explore the safety of insulin pump in the treatment of gestational diabetes mellitus(GDM).Methods A total of 106 patients with GDM admitted to Xiamen Humanity Maternity Hospital from February 2022 to February 2024 were selected as the research objects.According to different treatment methods,they were divided into reference group(multiple insulin injections)and observation group(insulin pump infusion),with 53 cases in each group.The levels of glucose metabolism[fasting blood glucose(FBG),2-hour postprandial blood glucose(2 hPBG),glycated hemoglobin A1c(HbA1c)],islet function[homeostasis model assessment-index resistance(HOMA-IR),homeostasis model assessment-β(HOMA-β)]and adverse reactions were compared between the two groups.Results Before delivery,the levels of HbA1c,FBG and 2 hPBG in the observation group were lower than those in the reference group,and the differences were statistically significant(all P<0.05).Before delivery,HOMAβin the observation group was higher than that in the reference group,and HOMA-IR was lower than that in the reference group,the differences were statistically significant(both P<0.05).The incidence of adverse reactions in the observation group was 1.89%(1/53),which was lower than 15.09%(8/53)in the reference group,and the difference was statistically significant(χ^(2)=5.950,P<0.05).Conclusion The application of insulin pump in the treatment of GDM patients can improve the effect of glucose control,improve islet function and reduce the risk of adverse reactions.
