摘要
目的探讨达格列净联合胰岛素泵强化治疗糖尿病(diabetes mellitus,DM)的临床疗效及其对胰岛β细胞功能、血糖控制的影响。方法选取2022年7月—2023年7月福建省三明市中西医结合医院收治的100例DM患者,按治疗方法不同分为两组,各50例。对照组采用胰岛素泵强化治疗,观察组采用达格列净联合胰岛素泵强化治疗。比较两组胰岛β细胞功能指数、首次达标时每日胰岛素用量、停泵前每日胰岛素用量、血糖达标时间及血糖指标。结果治疗后,观察组胰岛β细胞功能指数高于对照组,差异有统计学意义(P<0.05)。治疗后,观察组首次达标时每日胰岛素用量、停泵前每日胰岛素用量以及血糖达标时间均优于对照组,差异有统计学意义(P均<0.05)。治疗后,两组空腹血糖、餐后2 h血糖、糖化血红蛋白均下降,但两组血糖指标比较,差异无统计学意义(P均>0.05)。结论达格列净联合胰岛素泵强化治疗DM,可改善患者胰岛β细胞功能,且血糖控制效果更加理想,有助于患者血糖值趋于正常。
Objective To investigate the clinical efficacy of dapagliflozin combined with insulin pump in the treatment of diabetes mellitus(DM)and its effect on isletβcell function and blood glucose control.Methods From July 2022 to July 2023,a total of 100 DM patients admitted to Sanming Hospital of Integrated Traditional Chinese and Western Medicine were selected and divided into two groups according to different treatment methods,with 50 cases in each group.The control group was treated with insulin pump intensive treatment,and the observation group was treated with dapagliflozin combined with insulin pump intensive treatment.The homeostatic model assessment ofβ-cell function,daily insulin dosage at the time of first reaching the standard,daily insulin dosage before stopping the pump,time of blood glucose reaching the standard and blood glucose indexes were compared between the two groups.Results After treatment,the homeostatic model assessment ofβ-cell function of the observation group was higher than that of the control group,and the difference was statistically significant(P<0.05).After treatment,the daily insulin dosage at the first time of reaching the standard,the daily insulin dosage before stopping the pump and the time of blood glucose reaching the standard in the observation group were better than those in the control group,and the differences were statistically significant(all P<0.05).After treatment,fasting plasma glucose,2-hour postprandial plasma glucose and glycated hemoglobin A1c decreased in both groups,but there were no significant difference in blood glucose indexes between the two groups(all P>0.05).Conclusion Dapagliflozin combined with insulin pump intensive treatment of DM patients can improve the function of isletβcells,and the effect of blood glucose control is more ideal,which is helpful for the patient's blood glucose to return to normal.
