摘要
目的探讨甲状腺转录因子-1(TTF-1)、细胞角蛋白7(CK7)、p63、p40在非小细胞肺癌(NSCLC)患者中的表达及与分化程度的关系。方法选取107例NSCLC患者和100例肺部良性疾病患者,分别作为观察组和对照组。比较两组患者及不同分化程度NSCLC患者的TTF-1、CK7、p63、p40表达情况,并比较不同分化程度NSCLC患者的临床特征。采用Logistic回归模型分析NSCLC低分化的影响因素。结果观察组患者TTF-1、CK7阳性表达率均明显高于对照组,差异均有统计学意义(P﹤0.01)。低分化NSCLC患者TTF-1、CK7阳性表达率均明显高于中高分化患者,差异均有统计学意义(P﹤0.01)。低分化NSCLC患者肿瘤直径≥5 cm、淋巴结转移比例均明显高于中高分化患者,差异均有统计学意义(P﹤0.01)。多因素Logistic回归分析结果显示,TTF-1阳性表达、CK7阳性表达及淋巴结转移均是NSCLC低分化的危险因素(P﹤0.01)。结论NSCLC患者中TTF-1、CK7阳性表达率均较高,且TTF-1、CK7表达情况与NSCLC患者的分化程度有关。
Objective To investigate the expression of thyroid transcription factor-1(TTF-1),cytokeratin 7(CK7),p63,p40 in patients with non-small cell lung cancer(NSCLC)and their relationship with differentiation degree.Method A total of 107 patients with NSCLC and 100 patients with benign lung diseases were selected as observation group and control group,respectively.The expression of TTF-1,CK7,p63 and p40 in the two groups and NSCLC patients with different differentiation degree were compared,and the clinical characteristics of NSCLC patients with different differentiation degree were also compared.Logistic regression model was used to analyze the influencing factors of low differentiation of NSCLC.Result The positive expression rates of TTF-1 and CK7 in observation group were significantly higher than those in control group,and the differences were statistically significant(P<0.01).The positive expression rates of TTF-1 and CK7 in low-differentiated NSCLC patients were significantly higher than those in medium and high-differentiated patients,and the differences were statistically significant(P<0.01).The proportions of tumor diameter≥5 cm and lymph node metastasis in low-differentiated NSCLC patients were significantly higher than those in medium and high-differentiated patients,and the differences were statistically significant(P<0.01).Multivariate Logistic regression analysis showed that TTF-1 positive expression,CK7 positive expression,and lymph node metastasis were risk factors for low differentiation of NSCLC(P<0.01).Conclusion The positive expression rates of TTF-1 and CK7 are high in NSCLC patients,and the expression of TTF-1 and CK7 are related to the differentiation degree of NSCLC patients.
