摘要
目的构建基于循证的平衡力阶梯训练干预方案,并分析其在跌倒高风险管理中的作用。方法通过计算机检索跌倒高风险护理管理等相关证据,通过国际实践指南注册平台(http://www.guidelines-registry.cn)、美国国立临床诊疗指南数据库(NGC)等网站进行检索,检索文献经过小组进行筛选和整合,构建适合该疾病患者的平衡力阶梯训练模型。另以陕西省人民医院血管外科在2022年1月至2024年1月收治的96例跌倒高风险患者为研究对象,并随机分为对照组与观察组,各48例。对照组给予常规护理管理,观察组在常规基础上给予基于循证的平衡力阶梯训练。比较两组患者的运动功能、日常活动能力以及跌倒风险等级与跌倒事件发生率。结果干预30 d时,观察组Fugl-Meyer运动功能评定量表(FMA)评分、Barthel指数(BI)高于对照组;观察组的Morse跌倒评估量表(MFS)评分为(42.30±2.57)分与跌倒事件发生率为6.25%(3/48)低于对照组的(43.36±1.42)分、20.83%(10/48),差异均有统计学意义(均P<0.05)。结论本研究中构建的基于循证的平衡力阶梯训练可提升跌倒高风险患者的运动功能与日常活动能力,有效降低跌倒风险等级,减少跌倒事件的发生,效果良好。
Objective To construct an evidence-based balance force step training intervention program and analyze its role in the fall risk management.Methods The relevant evidence of nursing management of high risk of falls was retrieved by computer,and the international practice guideline registry platform(http://www.guidelines-registry.cn),the National Clinical Practice Guideline Database(NGC)and other websites were searched.After sifting and integrating literature retrieval by the team,a ladder training model of balance force suitable for patients with this disease was constructed.In addition,96 patients with high risk of fall admitted to the department of vascular surgery of Shaanxi Provincial People's Hospital from January 2022 to January 2024 were selected as the study object,and were randomly divided into a control group and an observation group,with 48 cases in each group.The control group was given routine nursing management,while the observation group was given evidence-based balance force step training on the basis of routine nursing.Motor function,daily activity ability,fall risk grade and fall event incidence were compared between the two groups.Results After 30 days of intervention,Fugl-Meyer Assessment(FMA)score and Barthel Index(BI)in the observation group were higher than those in the control group.The Morse Fall Scale(MFS)score of the observation group was(42.30±2.57)points and the incidence of falls was 6.25%(3/48),which were lower than those of the control group[(43.36±1.42)points and 20.83%(10/48)],and the differences were statistically significant(P<0.05).Conclusions The evidence-based balance force step training established in this study can improve the motor function and daily activity ability of patients at high risk of falls,effectively reduce the fall risk level,and reduce the occurrence of fall events,with good results.
