摘要
目的调查糖尿病性周围神经病理性疼痛(DPNP)患者疼痛灾难化(PC)现状,并分析其影响因素,为临床制订预防干预策略,提供参考依据。方法便利抽样法选取广西壮族自治区人民医院收治的206例DPNP患者为研究对象,采用一般资料调查表、疼痛数字等级评分量表(NRS)、疼痛灾难化量表(PCS)、领悟社会支持量表(PSSS)、糖尿病痛苦量表(DDS)对其进行调查。结果206例DPNP患者PC发生率44.66%(92/206),PCS总分为(30.10±5.16)分。多元线性回归分析结果显示,性别、糖尿病(DM)病程(≥10年)、多重用药、合并症数量(>5个)、NRS评分、PSSS评分、DDS各维度评分是PC的主要影响因素(均P<0.05),可解释PC总变异的92.3%。结论DPNP患者PC发生率较高,临床医护工作者应重视此类患者PC的评估,并根据主要影响因素制订科学有效、有针对性的干预措施,帮助患者减轻疼痛负担,以降低其PC水平。
Objective To investigate the status quo of pain catastrophizing(PC)in the patients with diabetic peripheral neuropathic pain(DPNP),and to analyze the influencing factors to provide reference for formulating clinical preventive intervention strategies.Methods A total of 206 patients with DPNP admitted and treated in the People’s Hospital of Guangxi Zhuang Autonomous Region were selected as the research subjects by convenience sampling method.The general data questionnaire,Numerical Rating Scale(NRS),Pain Catastrophizing scale(PCS),Perceived Social Support Scale(PSSS)and diabetes distress scale(DDS)were used to conduct the investigation.Results The incidence rate of PC in 206 cases of DPNP patients was 44.66%(92/206),and the total score of PCS was(30.10±5.16)points.The results of multiple linear regression analysis showed that the gender,duration of diabetes(≥10 years),multiple drug use,number of complications(>5),NRS score,PSSS score and scores of DDS dimensions were the main influencing factors of PC(all P<0.05),which could explain 92.3%of the total variation of PC.Conclusion The PC incidence rate in the patients with DPNP is high.Clinical healthcare workers should pay attention to the evaluation of PC in these patients,and formulate the scientific and effective targeted intervention measures according to the main influencing factors to help the patients to reduce the pain burden in order to reduce the level of PC.
